摘  要:目的探讨高山滑雪运动膝关节急性损伤机制及MRI表现。方法选取35例高山滑雪运动膝关节急性损伤患者作为实验组;随机选取31例普通膝关节急性损伤患者作为对照组。由2名放射科主治医师评估膝关节骨、软骨、半月板、韧带、肌腱、髌下脂肪垫及关节积液病变特点。采用χ^(2)检验及两独立样本非参数检验进行统计学分析。结果实验组的骨挫伤/骨折、软骨损伤、半月板损伤、韧带和肌腱损伤、髌下脂肪垫损伤发病率明显高于对照组(P<0.05),实验组和对照组关节腔积液发病率比较无统计学差异(χ^(2)=0.657,P=0.150)。实验组的关节软骨、半月板、韧带和肌腱损伤程度高于对照组(P<0.05)。MRI表现特点:实验组表现多个解剖部位骨挫伤/骨折,对照组表现单个解剖部位骨挫伤/骨折;实验组关节软骨常见≥Ⅲ级损伤,对照组关节软骨常见Ⅱ级损伤;实验组半月板主要表现Ⅱ/Ⅲ级损伤,对照组半月板主要表现Ⅰ/Ⅱ级损伤;实验组以多条韧带联合损伤为主,以Ⅱ级损伤居多,对照组以单条韧带损伤为主,以Ⅰ级损伤居多;实验组肌腱损伤常表现Ⅱ级损伤,对照组肌腱损伤常表现Ⅰ级损伤。结论高山滑血运动膝关节急性损伤主要表现为骨、软骨、半月板、韧带及肌腱的联合损伤。Ohjective To investigate the mechanism and MRI manifestations of acute knee injury in alpine sking.Methods 35 acute knee injuries in alpine skiing were collected as the experimental group.31 common acute knee injury were randomly selected as the control group.The pathological features of bone,cartilage.meniscus。ligament,tendon,subpatellar fat pad and joint effusion were evaluated by 2 attending radiologists.χ^(2)test and two independent samples test were used for statistical analysis.Results The incidence of bone contusion/fracture,carilage injury,meniscus injury,ligament and tendon injury,and subpatellar fat pad injury in the experimental group was significanly higher than that in the control group(P<0.05),and there was no significant dfference in the inidence of articular efusion between the experimental group and the control group(χ^(2)=0.657,P=0.150).The injury degroe of articular cartilage,meniscus。ligament and tendon in the experimental group was higher than that in the control group(P<0.05).MRI features,the experimental group showed bone contusion/fracture at multiple anatomical sites,and the control group showed bone contusion/fracture at single anatomical site;The experimental group often showed≥Ⅱ level injury of articular cartilage,and the control group often showed≥Ⅱlevel injury of articular crtilage;Meniscus were mainly showed Ⅱ/Ⅰlevel injury in experimental group,and Ⅰ/Ⅱ level injury in control group;The experimental group manifested more multiple ligament injury,in the majority with level Ⅱ,and the control group manifested a single ligament damage,in the majority with Ⅰ level;Tendon injuries often performed Ⅱ level in experimental group,and Ⅰ level in control group.Concusion Acute injury of the knee joint in alpine skiing mainly tmanifestes combined injury of bone,cartilage,meniscus。ligament,and tendon.
