摘  要:目的评估第3代双源CT单能量成像技术对颈动脉斑块显示的最佳单能量图像水平。方法回顾性选取275例患者的颈动脉斑块(30%<狭窄率<70%)重建出从40~150 keV单能量成像,间隔10 keV,获取275个斑块的单能量图像,进行主观及客观评价,并选取主观及客观指标较高的图像,与常规90 kVp/Sn150 kVp混合能量图像比较。结果与混合能量图像相比,在非钙化斑块、钙化斑块、混合斑块中,40~60 keV单能量图像的对比噪声比(CNR)、信噪比(SNR)均较高,并各组间均有统计学差异(P<0.05);3种类型斑块的主观评分,分别在40~90 keV、60~110 keV、60~70 keV的单能量图像较高,且各组间均有统计学差异(P<0.05)。结论与“常规”混合90 kVp/Sn150 kVp图像相比,评价颈动脉非钙化斑块、钙化斑块、混合斑块的最佳单能量图像水平分别为40~60 keV、60 keV、60 keV,其单能量图像可以明显改善颈动脉斑块的CNR、SNR及主观评分。Objective To evaluate the optimal level of monoenergetic image of carotid plaque by the third-generation dual-source CT monoenergetic imaging.Methods The carotid plaques(30%<stenosis rate<70%)of 275 patients were retrospectively ollected and rconstructed from 40 keV to 150 keV monoenergetic imaging.with an interval of 10 keV.275 plaques images were obtained for subjective and obictive evaluation,and images with higher subiective and obiective indicators were stlected for comparison with conventional 90 kVp/Sn150 kVp mixed energy images.Results Compared with mixed energy images,in non caleified plaques,calcified plaques and mixed plaques,the contrast-to noise ratio(CNR)and signal-to-noise ratio(SNR)of 40-60 keV monoenergetic images were all higher。and there were saistical dfferences among the groups(all P<0.05).The subiective scores of the three types plaques were higher in 40-90 keV,60-110 keV and 60-70 keV monoenergetic image,respectively,and there were statistical differences among the groups(all P<0.05).Conclusion Compared with the"conventional"mixed 90 kVp/Sn150 kVp image,the optimal monoenergetic image levels of non-caleified plaque,caleifiod paque and mixed plaque were evaluated at 40-60 keV,60 keV and 60 keV,respetively,and the monoenergetic image can significantly improve the CNR,SNR and subiective scores of carotid plaque.
