两种内固定方法治疗成人尺骨鹰嘴骨折的临床疗效对比

摘　　要：目的比较带孔折断式钛针与普通克氏针张力带治疗成人尺骨鹰嘴骨折的临床疗效。方法自2014年6月至2017年1月,我们对50例成人尺骨鹰嘴骨折患者行手术治疗,其中21例采用带孔折断式钛针张力带固定(A组),Mayo分型Ⅰ型8例、Ⅱ型13例;29例采用普通克氏针张力带固定(B组),May。分型Ⅰ型12例、Ⅱ型17例。两组患者一般资料差异无统计学意义(P>0.05)。比较两组患者的手术时间、术后疼痛评分、骨折愈合时间、内固定松动或失效、皮肤激惹情况及肘关节功能评分,并进行统计学分析。结果术后所有患者均获得随访,时间为12-20个月,平均15个月。两组患者的手术时间、术后疼痛评分、肘关节功能评分差异均无统计学意义(P>0.05),A组患者的内固定松动或失效、骨折愈合时间、皮肤激惹情况等均优于B组,差异有统计学意义(P<0.05)。结论带孔折断式钛针在不增加手术创伤的基础上,有助于成人尺骨鹰嘴骨折的愈合,并在减少内固定松动及皮肤激惹上优于普通克氏针。Objective To compare the clinical efficacy of breakable titanium needle with hole and ordinary Kirschner wire tension band in the treatment of olecranon fracture of ulna.Methods From June 2014 to January 2017,50 patients with olecranon fracture of ulna were perfbnned with surgical treatment.21 cases were fixed with breakable titanium needle tension band with hole(group A),8 cases o£Mayo typeⅠand 13 cases of Mayo typeⅡ.29 cases were fixed with ordinary Kirschner wire tension band(group B),12 cases of Mayo typeⅠand 17 cases of Mayo typeⅡ.There was no significant difference in general data between the two groups(P>0.05).The operation time,postoperative pain score,fracture healing time,loosening or failure of internal fixation,skin irritation and elbow joint function score of the two groups were compared and statistically analyzed.Results All the patients were follow-up for 12 to 20 months,with an average of 15 months.There was no significant difference in operation time,postoperative pain score and elbow joint function score between the two groups(P>0.05).The loosening or failure of internal fixation,fracture healing time and skin irritation in group A were better than those in group B(P<0.05).Conclusion On the basis of no increase of operative trauma,the breakable titanium needle tension band with hole is helpful to the healing of olecranon fracture of ulna in adults,and is superior to ordinary Kirschner wire tension band in reducing internal fixation looseness and skin irritation.
