上肢神经电损伤早期松解修复术后的临床疗效观察

摘　　要：目的探讨上肢神经电损伤早期显微外科松解修复的临床疗效和肌电图评估结果。方法自2013年4月至2019年12月,我们对8例肢体Ⅲ~Ⅳ度电击伤即上肢神经电损伤患者在伤后3个月内进行神经探查松解术。术前及术后2周、3个月行肌电图检查,术后6个月随访患肢感觉及运动功能恢复情况。结果术后2周本组患者行肌电图检查,结果显示受损神经支配肌均可记录到偶见或少量MUP,术后3个月肌电图结果显示神经功能进一步恢复。术后6个月随访,采用中华医学会手外科学会上肢功能评定试用标准评定患肢功能,均为优。结论采用早期显微外科松解修复上肢神经电损伤,能使受损神经功能得到有效恢复。Objective To investigate the clinical efficacy and electromyography evaluation results of early microsurgical release repair of upper limb peripheral nerve injury after electrical shock.Methods From April 2013 to December 2019,8 patients with upper limb peripheral nerve injury after electrical shock were treated with nerve exploration and release within 3 months after injury.They were Ⅲ to Ⅳ degree electrical injury.The electromyography was performed before operation,2 weeks and 3 months after operation.The recovery of sensory and motor function of affected limb was followed up 6 months after operation.Results Two weeks after operation,electromyography was performed in all the patients,and the results showed that the muscles innervated by injured nerve could be recorded occasionally or a small amount of MUP.The electromyography results showed that the nerve function was further recovered 3 months after operation.According to the evaluation standard of upper limb function of Hand Surgery Society of Chinese Medical Association,the follow-up results of the function of the affected limbs were excellent 6 months after operation.Conclusion The early microsurgical release on the electrical injured upper limb peripheral nerve can effectively restore the function of injured nerve.
