加味薏苡附子败酱散对复杂性肛瘘术后愈合影响研究

摘　　要：目的观察加味薏苡附子败酱散对复杂性肛瘘术后愈合的影响。方法将2018年8月—2019年6月在石家庄市中医院手术治疗的60例复杂性肛瘘患者随机分为治疗组和对照组各30例,术后对照组患者常规换药,治疗组在常规换药基础上给予加味薏苡附子败酱散口服,治疗15 d。观察2组患者术后不同时间点创面分泌物、疼痛、肉芽生长情况,比较2组治疗总有效率、创面愈合时间及不良反应发生情况。结果术后3 d 2组患者创面分泌物、疼痛、创面肉芽组织生长评分以及创面面积比较差异均无统计学意义(P均>0.05),随着时间延长,2组创面分泌物、疼痛、创面肉芽组织生长评分以及创面面积均明显下降(P均<0.05),各时间点时上述检测指标治疗组均明显低于对照组(P均<0.05);术后15 d,治疗组治疗总有效率为96.7%(29/30),明显高于对照组的73.4%(22/30),差异有统计学意义(P<0.05);治疗组创面愈合时间明显短于对照组(P<0.05);2组不良反应发生率比较差异无统计学意义(P>0.05)。结论加味薏苡附子败酱散可有效促进复杂性肛瘘术后患者创面愈合,缩短愈合时间,且安全。Objective It is to observe the effect of modified Yiyi Fuzi Baijiang Powder on the healing of complex anal fistulas after surgery.Methods Sixty patients with complex anal fistula surgically treated in Shijiazhuang Hospital of Traditional Chinese Medicine from August 2018 to June 2019 were randomly divided into treatment group and control group,with 30 cases in each group.The patients in the control group were routinely dressings after operation,and the patients in the treatment group was treated with modified Yiyi Fuzi Baijiang Powder by orally administration for 15 days based on the routinely dressings.The wound secretions,pain,and granulation growth of the patients in the two groups were observed at different time points after surgery,and the total effective rate of treatment,wound healing time and occurrence of adverse reactions were compared between the two groups.Results There was no significant difference in the scores of wound secretion,pain,wound granulation tissue growth and wound area between the two groups after in 3 days after operation(P>0.05).As time went by,the scores of wound secretions,pain,wound granulation tissue growth and wound area of the two groups were decreased significantly(P<0.05),and the above indicators at each time point in the treatment group were significantly lower than those in the control group(P<0.05));the total effective rate of treatment in the treatment group was 96.7%(29/30)in 15 days after operation,which was significantly higher than that 73.4%(22/30)in the control group,the difference was significant(P<0.05);the wound healing time of the treatment group was significantly shorter than that of the control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Modified Yiyi Fuzi Baijiang Powder can effectively promote wound healing and shorten the healing time in patients with complex anal fistula after surgery,and its safety is good.
