摘 要:目的:探讨基于可视化技术的经皮肾镜取石术(PCNL)对肾结石患者取石效果及肾功能的影响。方法:选取2017年2月~2019年2月本院肾结石患者136例,依据随机表分为可视化组(n=68)和PCNL组(n=68),PCNL组给予常规PCNL治疗,可视化组给予基于可视化技术的PCNL治疗,比较两组围术期指标、取石效果、肾功能\[血尿素氮(BUN)、血肌酐(SCr)\]、并发症。结果:可视化组术中出血量、通道建立时间、住院时间明显低于PCNL组,差异有统计学意义(P<0.05),可视化组和PCNL组手术时间、复发率、结石清除率比较差异无统计学意义(P>0.05);可视化组和PCNL组术后BUN、SCr明显高于术前,可视化组术后BUN、SCr明显低于PCNL组,差异有统计学意义(P<0.05);可视化组并发症率明显低于PCNL组(P<0.05)。结论:基于可视化技术的PCNL治疗肾结石患者具有良好的取石效果,可有效减少手术创伤及并发症,有利于减轻患者肾功能损伤,值得临床推广。Objective:To discuss the effect of percutaneous nephrolithotripsy(PCNL)based on visualization technology on the stone removal and renal function in the patients with kidney stone.Method:From February 2017 to February 2019,136 patients with kidney stones in our hospital were selected,and they were randomly divided into the visualization group(n=68)and PCNL group(n=68).The PCNL group was given routine PCNL treatment,while the visualization group was given PCNL treatment based on visualization technology.The perioperative indexes,stone taking effect,renal function[blood urea nitrogen(BUN),serum creatinine(SCR)]and complications were compared between the two groups.Result:The intraoperative bleeding volume,channel establishment time and hospitalization time in the visualization group were significantly lower than those in the PCNL group,and the difference was statistically significant(P<0.05).However,there was no significant difference in operation time,recurrence rate or stone clearance rate between the visualization group and PCNL group(P>0.05).The BUN and SCR in visualization group and PCNL group after operation were significantly higher than those in the before operation.The BUN and SCR in the visualization group after operation were significantly lower than those in the PCNL group,and the difference was statistically significant(P<0.05).The complication rate in the visualization group was significantly lower than that in the PCNL group(P<0.05).Conclusion:PCNL based on visualization technology has the good effect on the treatment of patients with kidney stone.It can effectively reduce the surgical trauma and complications,reduce the renal function damage of patients,so it’s worth further clinical promotion.
