摘　　要：目的探讨E-钙粘蛋白(E-cadherin)和降钙素原(PCT)对急性胰腺炎(AP)患者严重程度的早期诊断价值。方法选取临床诊断为AP患者76例,收集患者基本临床资料,选定病程不同采样点(2 h和24 h),ELISA法检测外周血E-cadherin、降钙素原(PCT)表达,48 h后细分为轻症胰腺炎组(MAP组)、中度重症胰腺炎组(MSAP组)、重症胰腺炎组(SAP组),比较其不同时间点的结果差异。结果 PCT结果提示在病情发生12 h MAP组、MSAP组及SAP组两两比较差异均无统计学意义(P>0.05);在病情发生24 h MAP组、MSAP组及SAP组两两比较差异均有统计学意义(P<0.05)。E-cadherin结果显示在病情发生早期(12 h)血浆E-cadherin即明显升高,且MAP组、MSAP组及SAP组在不同时间点两两比较差异均有统计学意义(P<0.05)。结论 PCT在病情发生24 h后对病情严重程度的判断具有临床价值,但在病情早期(24 h前)预测价值有限。而E-cadherin的检测对AP早期病情严重程度的判断具有重要临床价值。Objective To explore the early diagnostic value of E-cadherin in the patients with acute pancreatitis(AP) and procalcitonin.Methods Clinical data of 76 patients with AP were collected;the serum levels of E-cadherin and PCT were measured at different sampling points(12 hours and 24 hours).The patients were divided into the MAP group,MSAP group and SAP group at 48 hours,and then compared the diagnostic value at different time points.Results The detection values of PCT had no significant difference between MAP group,MSAP group and SAP group at 12 hours after the onset of the disease(P>0.05),but there were significantly differences between MAP group,MSAP group and SAP group after 24 hours.The serum E-cadherin in MSAP group and SAP group were significantly higher than that in MAP group,and there were significant difference between MSAP group and SAP group at different sampling points(12 and 24 hours)(P<0.05).Conclusions PCT had clinical value in judging the severity of the disease at 24 hours after the onset of the disease,but their predictive value was limited in the early stage of the disease(before 24 hours).The detection of E-cadherin had important clinical value in judging the severity of AP in early stage.
