新生儿凝血功能异常的危险因素分析

摘　　要：目的探讨新生儿凝血功能异常的危险因素,为早期识别凝血功能异常提供依据。方法选取130例新生儿作为研究对象,收集其凝血功能指标及临床资料,根据凝血功能结果分为异常组(70例)和正常组(60例)。对可能导致新生儿凝血功能异常的危险因素进行单因素分析和非条件多因素Logistic回归分析。结果单因素分析显示,凝血功能异常组与正常组在宫内窘迫、妊娠期高血压、胎盘早剥、新生儿呼吸窘迫综合征(NRDS)、新生儿肺炎、新生儿酸中毒发生率比较,差异有统计学意义(P<0.05);非条件多因素Logistic回归分析显示,NRDS(OR=5.095,95%CI:1.246-20.826)、新生儿肺炎(OR=3.023,95%CI:1.332-6.856)为新生儿凝血功能异常的独立危险因素。结论 NRDS、新生儿肺炎可导致新生儿凝血功能异常。Objective To explore the risk factors for abnormal coagulation function in neonates,and in order to provide evidence for early identification of coagulation dysfunction.Methods The clinical data and coagulation function indexes of 130 newborns were collected.According to the results of coagulation function,they were divided into abnormal group(70 cases)and normal group(60 cases).Univariate multivariate Logistic regression analysis methods were used to summarize the risk factors of neonatal coagulation dysfunction.Results The univariate analysis showed that there were significant differences between two groups in fetal distress,pregnancy induced hypertension,placental abruption,neonatal respiratory distress syndrome,neonatal pneumonia,and neonatal acidosis(P<0.05).The unconditional multivariate Logistic regression analysis showed that NRDS(OR=5.095,95%CI:1.246-20.826),neonatal pneumonia(OR=3.023,95%CI:1.332-6.856)were independent risk factors for abnormal coagulation function in newborns.Conclusion NRDS and neonatal pneumonia may lead to abnormal coagulation function in newborns.
