摘  要:目的探讨外周血单核细胞与大血小板的比率(MLPR)联合D-二聚体对慢阻肺急性加重期(AECOPD)患者继发肺栓塞(PTE)的预测价值。方法选取2017年1月~2019年12月于潍坊医学院附属医院住院的AECOPD合并PTE患者30例作为研究组,另选取同时期住院的AECOPD患者30例作为对照组。对所有入选的患者行单核细胞与大血小板的比率(MLPR)、血浆D-二聚体检查并进行分析。结果与AECOPD组相比,AECOPD合并PTE组患者血清D-二聚体、MLPR明显升高,差异有统计学意义(P<0.05)。D-二聚体、MLPR联合D-二聚体对AECOPD合并PTE的ROC曲线下面积(AUC)分别为0.811,0.919。MLPR联合D-二聚体的敏感度为90.1%,特异性为87.6%,均大于单独D-二聚体。结论 (1)血清D-二聚体、MLPR在慢阻肺合并肺栓塞患者中明显升高。(2)MLPR联合D-二聚体可提高预测AECOPD合并PTE的敏感度及特异性,预测价值比单一检测D-二聚体更大。Objective To investigate the predictive value of peripheral blood mononuclear cells to large platelet ratio(MLPR)combined with D-dimer in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and pulmonary thromboembolism(PTE).Methods Thirty patients with AECOPD and PTE were selected as the research group,and 30 patients with AECOPD were selected as the control group.The monocyte-to-large platelet ratio(MLPR),plasma D-dimer were measured and compared in the 2 groups.Results The serum D-dimer and MLPR in the research group were higher than that in the control group(P<0.05).The area under the ROC curve of D-dimer,MLPR combined with D-dimer in the research group were 0.811 and 0.919,respectively.The sensitivity of D-dimer combined with MLPR was 90.1%,and the specificity was 87.6%,both of which were greater than D-dimer alone.Conclusion(1)Serum D-dimer and MLPR are significantly increased in patients with AECOPD and PTE.(2)MLPR combined with D-dimer can improve the sensitivity and specificity of predicting AECOPD combined with PTE,and the predictive value is greater than that of single detection of D-dimer.
