高渗糖胸膜腔注射联合胸膜摩擦在胸腔镜肺大疱切除术中的临床应用

摘　　要：目的探讨高渗糖胸膜腔注射联合胸膜摩擦在胸腔镜肺大疱切除术中的临床应用。方法选取2018年5月~2019年8月接受胸腔镜下肺大疱切除术的自发性气胸患者60例,随机分成两组,对照组给予单纯胸膜摩擦,研究组给予高渗糖胸膜腔注射联合胸膜摩擦治疗,比较两组患者手术指标和并发症发生情况。结果研究组引流天数、住院时间均少于对照组(P<0.05);且研究组并发症总发生率显著低于对照组(P<0.05)。2组手术时间、术中出血量、术后第3天疼痛评分比较,差异无统计学意义(P>0.05)。结论在胸腔镜肺大疱切除术中采用高渗糖胸膜腔注射联合胸膜摩擦治疗能够有效防止术后肺漏气,促进肺复张,缩短术后带管时间,减少住院时间,促进术后康复。Objective To investigate the clinical application of hyperosmotic glucose injected into the pleural cavity combined with pleural friction in the treatment of bullous pulmonary resection by thoracoscopy.Methods From May 2018 to August 2019,60 patients with spontaneous pneumothorax who received thoracoscopic bullectomy were randomly divided into two groups,the control group was given simple pleural friction,and the observation group was given hyperosmotic glucose injected into the pleural cavity combined with pleural friction treatment.The indexes and complications were compared between the two groups.Results The drainage days and hospital stay in the study group were less than those in the control group(P<0.05),and the total incidence rate of complications in the study group was significantly lower than that in the control group(P<0.05).There was no significant difference between the two groups in operation time,intraoperative blood loss and postoperative pain score on the third day(P>0.05).Conclusion The combination of hyperosmotic glucose injected into the pleural cavity and pleural friction can effectively prevent postoperative pulmonary air leakage,promote the re-expansion of the lung,shorten the postoperative time with tube,reduce the hospital stay and promote the postoperative rehabilitation.
