摘  要:目的观察温和灸预处理对幼鼠心肌缺血再灌注损伤(MIRI)的影响,探讨其作用机制。方法将40只SD幼鼠采用随机数字表法分为模型组、假手术组、缺血预适应组、温和灸预处理组,每组10只。模型组、假手术组、缺血预适应组每日捆绑20 min,温和灸预处理组灸内关、足三里、关元,每日1次,连续7 d。第8天,通过可逆性左冠状动脉前降支结扎法复制心肌缺血再灌注模型,假手术组仅穿线不结扎,缺血预适应组予心肌缺血预适应处理。造模结束后取材,检测血清肌酸激酶同工酶(CK-MB)、心肌肌钙蛋白T(cTnT)、三磷酸腺苷(ATP)和超氧化物歧化酶(SOD)水平,HE染色法和透射电镜观察缺血区心肌组织病理形态和超微结构改变。结果与假手术组比较,模型组血清CK-MB、cTnT浓度上升(P<0.05),ATP含量和SOD活性下降(P<0.05);与模型组比较,缺血预适应组和温和灸预处理组血清CK-MB、cTnT浓度显著降低(P<0.05),ATP含量和SOD活性显著增加(P<0.05);温和灸预处理组和缺血预适应组比较,差异无统计学意义(P>0.05)。HE染色和透射电镜结果显示,假手术组心肌纤维轻微肿胀,细胞结构无明显异常;模型组心肌肌纤维溶解断裂,结构缺损,线粒体嵴不规则,自噬空泡较多;与模型组相比,缺血预适应组和温和灸预处理组心肌肿胀、坏死较轻,细胞核和线粒体结构基本完整,自噬空泡减少。结论温和灸预处理能保护MIRI幼鼠的心功能,并产生与心肌缺血预适应相似的减轻MIRI的效应,作用机制可能与减轻心肌细胞线粒体自噬,维持心肌能量供应,增强心肌抗氧化能力有关。Objective To observe the effect of pretreatment with mild moxibustion on myocardial ischemiareperfusion injury(MIRI)in young rats,and to discuss its action mechanism.Method Forty young Sprague-Dawley(SD)rats were divided into a model group,a sham operation group,an ischemic preconditioning group and a mild moxibustion pretreatment group,with 10 rats in each group.Rats in the model,sham operation and ischemic preconditioning groups were bound for 20 min each day,while rats in the mild moxibustion pretreatment group were given moxibustion at Neiguan(PC6),Zusanli(ST36)and Guanyuan(CV4),once a day for 7 consecutive days.On the eighth day,the myocardial ischemia-reperfusion model was replicated via reversible ligation of the left anterior descending coronary artery;the suture was only threaded but not ligated in the sham operation group;myocardial ischemic preconditioning was conducted in the ischemic preconditioning group.Samples were collected after modeling.Serum creatine kinase isoenzyme MB(CK-MB),cardiac troponin T(cTnT),adenosine triphosphate(ATP)and superoxide dismutase(SOD)were detected,and the pathomorphology and ultrastructure of the ischemic myocardiac tissues were observed using hematoxylin-eosin(HE)staining under transmission electron microscope.Result Compared with the sham operation group,the concentrations of serum CK-MB and cTnT increased (P<0.05) and the content of ATP and activity of SOD decreased in the model group (P<0.05);comparedwith the model group, the concentrations of serum CK-MB and cTnT decreased significantly (P<0.05) and the contentof ATP and activity of SOD increased significantly (P < 0.05) in the ischemic preconditioning groupand mild moxibustion pretreatment group;there were no significant differences between the mild moxibustionpretreatment group and the ischemic preconditioning group (P > 0.05). HE staining and transmissionelectron microscope revealed that the myocardial fibers were slightly swollen while the cell structure wassubstantially normal in the sham operation group;in the
