补肾固冲止血方治疗围绝经期功能失调性子宫出血临床研究

摘　　要：目的观察补肾固冲止血方联合西药治疗围绝经期功能失调性子宫出血的临床疗效,旨在为更好的治疗围绝经期功能失调性子宫出血患者提供有效的理论依据。方法将我院收治的98例围绝经期功能失调性子宫出血患者随机分为对照组与研究组各49例,对照组患者采用米非司酮片联合刮宫术治疗,研究组患者在对照组治疗基础上联合补肾固冲止血方治疗。观察并比较不同治疗方式对两组患者临床疗效、性激素水平、出血量、出血原因、月经周期评分及子宫内膜厚度、不良反应情况的影响。结果治疗后,研究组患者总有效率显著高于对照组(P<0.05);两组性激素促卵泡生成素(FSH)、雌激素(E2)、黄体生成素(LH)水平均较治疗前明显降低,且研究组降低程度大于对照组(P<0.05);两组患者出血量、出血原因、月经周期评分均较治疗前明显降低,且研究组降低程度大于对照组(P<0.05);两组患者子宫内膜厚度均较治疗前明显降低,且研究组患者降低程度大于对照组(P<0.05);两组不良反应情况比较无差异(P>0.05)。结论补肾固冲止血方联合西药治疗能明显改善围绝经期功能失调性子宫出血患者中医症候疗效、性激素以及一般临床症状,且安全性高,复发率较低,值得临床进一步推广运用。Objective To observe the clinical efficacy of Bushen Guchong Zhixue Decoction combined with western medicine in treatment of perimenopause dysfunctional uterine bleeding,and provide an effective theoretical basis for the better treatment. Methods 98 patients with perimenopausal dysfunctional uterine bleeding were randomly divided into control group and study group,49 cases each. Control group was treated with Mifepristone Tablets combined with curettage;study group was treated with Bushen Guchong Zhixue Decoction on the basis of control group. The clinical efficacy,sex hormone level,bleeding volume,bleeding causes,menstrual cycle score,endometrial thickness,and recurrence rate were compared between groups. Results After treatment,the total effective rate of study group was significantly higher than that of control group(P<0.05);the sex hormone follicle-stimulating hormone(FSH),estrogen(E2),Luteinizing hormone(LH)levels of both groups decreased,and were more significant in study group than that in control group(P<0.05);the scores of bleeding volume,bleeding causes and menstrual cycle decreased after treatment,and were more significant in study group than that in control group(P<0.05);the endometrial thickness decreased after treatment in both groups,and were more significant in study group than that in control group(P<0.05);there was no difference in the incidence rate between the two groups(P>0.05). Conclusion Bushen Guchong Zhixue Decoction combined with western medicine can effectively and safely ameliorate the TCM symptoms,sex hormone levels and general clinical symptoms of perimenopause dysfunctional uterine bleeding patients with lower recurrence rate,which is worthy of further clinical application.
