腰椎间盘突出症血清TNF-α、MMP-2表达水平及其与中医证型的关系

摘　　要：目的观察腰椎间盘突出症血清肿瘤坏死因子(TNF-α)、基质金属蛋白酶-2(MMP-2)表达水平及其与中医证型的关系。方法选取本院收治的120例腰椎间盘突出症患者,根据中医辨证分型分为气滞血瘀组与肝肾亏虚组各60例,采用酶联免疫法检测两组血清TNF-α、MMP-2表达水平,分析不同中医证型腰椎间盘突出症患者血清TNF-α、MMP-2的表达差异。结果治疗后,气滞血瘀组患者血清TNF-α、MMP-2水平显著低于肝肾亏虚组患者(P<0.05);气滞血瘀组患者腰痛视觉模拟评分(VAS)和Oswestry功能障碍指数(ODI)评分均低于肝肾亏虚组患者(P<0.05);Pearson相关系分析结果显示,腰椎间盘突出症患者血清TNF-α水平与VAS、ODI评分呈正相关;腰椎间盘突出症患者血清MMP-2水平与VAS、ODI评分呈正相关,差异均有统计学意义(P<0.05)。结论气滞血瘀型腰椎间盘突出症患者与肝肾亏虚型患者相比,血清TNF-α水平更高、MMP-2水平更低,二者与VAS、ODI评分有相关性,可作为中医辨证的辅助指标。Objective To observe the expression levels of serum tumor necrosis factor-α(TNF-α)and matrix metalloproteinase-2(MMP-2)and their relationship with TCM syndromes in lumbar disc herniation(LDH). Methods A total of 120 LDH patients were divided into qi-stagnation blood stasis group and liver-kidney deficiency group according to different TCM differentiation syndromes,60 cases each. The expression levels of serum TNF-αand MMP-2 in both groups were detected by enzyme-linked immunosorbent assay(ELISA). The differences in expression of serum TNF-α and MMP-2 in LDH patients with different TCM syndromes were analyzed. Results The levels of serum TNF-α and MMP-2 in qi-stagnation blood stasis group were were lower than those of liver-kidney group(P<0.05);the scores of Visual Analog Scale(VAS)and Oswestry Disability Index(ODI)in qi-stagnation blood stasis group were lower than those of liver-kidney deficiency group(P<0.05);the results of Pearson correlation analysis showed that level of serum TNF-αwas positively correlated with scores of VAS and ODI,and level of serum MMP-2 was positively correlated with scores of VAS and ODI in LDH patients(P<0.05). Conclusion Compared with LDH patients of liver-kidney deficiency,the level of serum TNF-α in LDH patients of qi-stagnation blood stasis is higher,while the level of MMP-2 is lower,which are related to the scores of VAS and ODI,it can be applied as auxiliary indexes of TCM syndromes.
