摘 要:目的评估补充omega-3多不饱和脂肪酸(omega-3 PUFA)对2型糖尿病(T2DM)患者空腹血糖和糖化血红蛋白的影响。方法 :检索PubMed,Elsevier,Cochrane Library数据库,搜集1988-2019年符合条件的随机对照试验(RCT)。数据以标准化均数差(SMD)及95%置信区间(95%CI)表示,应用Rveman5.3统计软件,采用固定效应模型进行分析。结果本文共纳入26例RCT试验。在合并分析中,与对照组相比补充omega-3 PUFA对T2DM患者空腹血糖(FBG)(SMD=0.09,95%CI:-0.01,0.20;12=57%)以及糖化血红蛋白(HbA1c)(SMD=0.01,95%CI:0.10,-0.11;I2=48%)无显著影响。在FBG亚组分析中发现,T2DM患者血糖基线水平、补充omega-3 PUFA时间均显著影响T2DM患者FBG水平;在HbA1c亚组分析中,T2DM患者HbA1c基线水平、日补充omega-3 PUFA剂量以及干预时间均对T2DM患者HbA1c水平具有显著影响。结论长期膳食补充omega-3 PUFA可能会对T2DM患者空腹血糖水平起到积极作用,但需要根据自身血糖及HbA1c基线水平持续12 w以上补充合适剂量的Omega-3 PUFA。Objective To evaluate the effect of omega-3 polyunsaturated fatty acids(Omega-3 PUFA) supplementation on blood glucose/glycated hemoglobin in patients with type 2 diabetes mellitus(T2 DM).Methods PubMed,Elsevier,and Cochrane Library databases were searched from 1988 to 2019 for randomized controlled trials(RCT) investigating the effect of omega-3 PUFA supplementation on glucose control in patients with type 2 diabetes.Data were pooled and analyzed using RveMan 5.3 statistical software with fixed effects model,and expressed as standardized mean difference(SMD) with 95%confidence interval(95% CI).Results A total of 26 RCT trials were included.In the combined analysis,omega-3 PUFA supplementation had no significant effect on fasting blood glucose(FBG)(SMD=0.09,95% CI:-0.01,0.20;I^2=57%) and glycosylated hemoglobin(HbA1 c)(SMD=0.01,95% CI:0.10,-0.11;I^2=48%).In the FBG subgroup analysis,baseline blood glucose levels and omega-3 PUFA supplementation in T2 DM patients significantly affected FBG levels in T2 DM patients.Conclusion Supplementation of omega-3 PUFA may improve glucose control in patients with T2 DM,but it is recommended to supplement omega-3 PUFA at the appropriate dose based on their own blood glucose and HbAlc baseline levels.[ACTA NUTRIMENTA SINICA,2020,42(5):449-457]
