交锁髓内钉固定与加压钢板螺钉固定治疗肱骨骨折后骨不连的效果  

Effect of Interlocking Intramedullary Nail Fixation and Compression Plate Screw Fixation in the Treatment of Nonunion After Humeral Fracture

摘　　要：目的比较交锁髓内钉固定与加压钢板螺钉固定治疗肱骨骨折后骨不连的临床疗效。方法选取2016年10月~2020年10月我院诊治的52例肱骨骨折后骨不连患者为研究对象,采用随机数字表法分为对照组和观察组,各26例。对照组采用加压钢板螺钉固定,观察组采用交锁髓内钉固定,比较两组临床治疗疗效、手术指标、骨折愈合时间、肩关节功能Neer评分以及并发症发生率。结果观察组治疗优良率为96.15,高于对照组的92.30%,差异无统计学意义(P>0.05);观察组手术时间、术中出血量、住院时间、骨折愈合时间均短于对照组,差异有统计学意义(P<0.05);治疗后,观察组肩关节功能Neer评分高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为7.69%,低于对照组的19.23%,差异有统计学意义(P<0.05)。结论交锁髓内钉固定与加压钢板螺钉固定治疗肱骨骨折后骨不连均可获得良好治疗效果,但是交锁髓内钉固定手术时间短,术中出血量少,利于患者术后恢复,可缩短住院时间和骨折愈合时间,提高肩关节功能Neer评分,降低并发症发生率。Objective To compare the clinical effects of interlocking intramedullary nail fixation and compression plate screw fixation in the treatment of nonunion after humeral fracture.Methods 52 patients with nonunion after humeral fracture treated in our hospital from October 2016 to October 2020 were selected as the research objects.They were divided into control group and observation group by random number table method,with 26 cases in each group.The control group was fixed with compression plate screws,and the observation group was fixed with interlocking intramedullary nails.The clinical treatment effects,surgical indicators,fracture healing time,Neer score of shoulder joint function,and complication rates were compared between the two groups.Results The excellent and good rate of treatment in the observation group was 96.15%,which was higher than 92.30%in the control group,the difference was not statistically significant(P>0.05);The operation time,intraoperative blood loss,hospitalization time,and fracture healing time of the observation group were shorter than those of the control group,the difference was statistically significant(P<0.05);After treatment,the Neer score of shoulder joint function in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05);The complication rate in the observation group was 7.69%,which was lower than 19.23%in the control group,the difference was statistically significant(P<0.05).Conclusion Both interlocking intramedullary nail fixation and compression plate screw fixation for the treatment of nonunion after humeral fractures can achieve good results.However,the operation time of interlocking intramedullary nail fixation is short and the amount of intraoperative blood loss is small,which is beneficial to the patient's postoperative recovery.It can shorten the hospital stay and fracture healing time,improve the Neer score of shoulder joint function,and reduce the incidence of complications.
