摘  要:目的探讨孕期体重指数(BMI)及血脂指标与妊娠期高血压及子痫前期的相关性。方法选择2018年1月-2019年12月于我院分娩的妊娠期高血压疾病产妇115例(其中妊娠期高血压63例,子痫前期52例)作为研究组。另外选取于我院分娩的健康产妇115例作为对照组。比较两组孕产妇BMI及血脂指标差异,并进行多因素分析。结果研究组产妇孕前BMI[(22.89±3.96)kg/m^(2) vs(20.19±4.01)kg/m^(2)]、建卡时BMI[(23.61±4.72)kg/m^(2) vs(21.72±4.11)kg/m^(2)]、孕37wBMI[(26.72±5.33)kg/m^(2) vs(23.72±5.10)kg/m^(2)]、建卡时TC[(4.78±0.99)mmol/L vs(4.39±0.87)mmol/L]、建卡时TG[(1.56±0.63)mmol/L vs(1.23±0.51)mmol/L]、建卡时HDL[(1.67±0.41)mmol/L vs(1.85±0.35)mmol/L]、建卡时LDL[(2.48±0.93)mmol/L vs(2.12±0.59)mmol/L]、孕37wTCL[(6.69±1.37)mmol/L vs(6.19±1.13)mmol/L]、孕37w TGL[(3.80±1.65)mmol/L vs(3.02±1.39)mmol/L]与对照组相比有显著差异(P<0.05),以上指标均是影响产妇发生妊娠期高血压疾病的因素。研究组产妇孕37wHDL、孕37wLDL与对照组相比无显著差异(P<0.05)。以研究组作为因变量,以单因素分析具有统计学意义的指标作为自变量,并对各指标进行变量赋值,带入Logistic模型中进行多元回归分析,结果显示孕前BMI(OR=1.167,P=0.025)、建卡时BMI(OR=1.715,P=0.003)、孕37wBMI(OR=1.453,P=0.011)、建卡时TC(OR=2.956,P=0.001)、建卡时TG(OR=1.160,P=0.022)、建卡时HDL(OR=1.1522,P=0.000)、建卡时LDL(OR=1.657,P=0.015)、孕37wTC(OR=1.706,P=0.003)、孕37wTG(OR=1.502,P=0.004)均是影响孕产妇妊娠期高血压及子痫前期发生的独立因素。结论妊娠期高血压及子痫前期产妇BMI及血脂指标与正常孕妇有显著差异,BMI及血脂指标的检测对于妊娠期高血压疾病的发生有一定的预测价值。Objective:To explore the correlation between pregnancy body mass index(BMI)and blood lipid index and pregnancy hypertension and preeclampsia.Methods:115 pregnant women with hypertensive disorder complicating pregnancy(63 pregnant women with hypertension and 52 pre eclampsia)who were delivered in our hospital from January 2018 to December 2019 were selected.In addition,115 healthy parturients in our hospital were selected as the control group.The difference of BMI and blood lipid index between the two groups was compared,and multi factor analysis was carried out.Results:Compared with the control group,there were significant differences in BMI before pregnancy[(22.89±3.96)kg/m^(2) vs(20.19±4.01)kg/m^(2)],BMI at card establishment[(23.61±4.72)kg/m^(2) vs(21.72±4.11)kg/m^(2)],BMI at 37 weeks of pregnancy[(26.72±5.33)kg/m^(2) vs(23.72±5.10)kg/m^(2)],TC at card establishment[(4.78±0.99)mmol/L vs(4.39±0.87)mmol/L],TG at card establishment[(1.56±0.63)mmol/L vs(1.23±0.51)mmol/L,HDL at card establishment[(1.67±0.41)mmol/L vs(1.85±0.35)mmol/L],LDL at card establishment[(2.48±0.93)mmol/L vs(2.12±0.59)mmol/L],TC at 37 weeks of pregnancy[(6.69±1.37)mmol/L vs(6.19±1.13)mmol/L]and TG at 37 weeks of pregnancy[(3.80±1.65)mmol/L vs(3.02±1.39)mmol/L]in the study group.Taking the research group as the dependent variable,the indexes with statistical significance in single factor analysis as the independent variable,and assigning the variable value to each index,and taking them into the logistic model for multiple regression analysis,the results showed that BMI before pregnancy(OR=1.167,P=0.025),BMI at card establishment(OR=1.715,P=0.003),BMI at 37 weeks of pregnancy(OR=1.453,P=0.011),TC at card establishment(OR=2.956,P=0.001),TG at card establishment(OR=1.160,P=0.022),HDL at card establishment(OR=1.1522,P=0.000),LDL at card establishment(OR=1.657,P=0.015),TC at 37 weeks of pregnancy(OR=1.706,P=0.003)and TG at 37 weeks of pregnancy(OR=1.502,P=0.004)all affected the pregnant women′s pregnancy height Independent facto
