摘  要:目的探讨超声心动图在窒息新生儿心肌损害评估中的价值。方法回顾性分析2014年01月至2019年01月我院收治的80例窒息新生儿超声心动图结果,根据窒息程度分为轻度窒息组(4≤1min Apgar评分≤7,n=40)和重度窒息组(0<1min Apgar评分≤3,n=40),选取同期40例正常足月新生儿作为对照组,对超声心动图结果和三组心肌生化指标进行比较分析。结果三组新生儿左、右心室E、A、E/A、EF比较差异无统计学意义(P>0.05);重度窒息组和轻度窒息组左、右心室Tei指数、ICT、IRT和AST、LDH、CK、CK-MB水平均显著高于对照组,ET低于对照组(P<0.05),重度窒息组左、右心室Tei指数、ICT、IRT和AST、LDH、CK、CK-MB水平均显著高于轻度窒息组(P<0.05),ET比较差异无统计学意义(P>0.05);Spearman相关性分析显示,AST、LDH、CK、CK-MB水平与E、A、E/A、EF、Tei指数、ICT、IRT及ET 呈正相关性(P<0.05)。结论 Tei指数、ICT、IRT能够评估窒息新生儿心脏受损程度,为治疗提供依据。Objective:To explore the value of echocardiography in the assessment of myocardial damage in asphyxiated neonates.Methods:The echocardiographic results of 80 cases of neonatal asphyxia admitted to our hospital from January 2014 to January 2019 were retrospectively analyzed.According to the degree of asphyxia,the patients were divided into the mild asphyxia group(4≤1min Apgar score ≤7,n=40)and the severe asphyxia group(0<1min Apgar score ≤3,n=40).40 normal full-term newborns in the same period were selected as the control group,and the results of echocardiography and myocardial biochemical indexes in the three groups were compared and analyzed.Results:There was no significant difference in left and right ventricle E,A,E/A and EF between the three groups(P>0.05).Tei index,ICT,IRT and AST,LDH,CK and CK-MB levels in the severe asphyxia group and the mild asphyxia group were significantly higher than those in the control group,while ET levels were lower than those in the control group(P<0.05).The levels of Tei,ICT,IRT,AST,LDH,CK and ck-mb in the severe asphyxia group were significantly higher than those in the mild asphyxia group(P<0.05),and the differences in ET were not statistically significant(P>0.05).Spearman correlation analysis showed that AST,LDH,CK and CK-MB were positively correlated with E,A,E/A,EF,Tei index,ICT,IRT and ET(P<0.05).Conclusion:Tei index,ICT and IRT can be used to assess the extent of heart damage in newborns with asphyxia,providing a basis for assessing the condition and treatment.
