摘　　要：目的研究冷冻单囊胚复苏移植术中胚胎气液滴位置距宫底的距离与妊娠结局的关系。方法回顾性分析2012年1月1日至2017年12月31日3077例冷冻单囊胚复苏移植患者,根据胚胎移植时腹部彩超测量胚胎气液滴位置距宫底的距离的不同分为4组:D1:<1cm(n=2016)、D2:1-1.5cm(n=902)、D3:1.51-1.9cm(n=124)、D4:>1.9cm 4组(n=35),比较不同组的妊娠结局。结果从D1组到D4组的妊娠率分别为55.5%、48.8%、42.7%、31.4%,活产率分别为46.5%、40.6%、34.7%、25.7%,表现出逐步降低的趋势(P<0.01)。在校正了混杂因素后,D2、D3、D4各组妊娠率相对于D1组的OR(95%CI)分别为0.74(0.63-0.87)、0.58(0.4-0.84)、0.35(0.17-0.72);活产率相对于D1组的OR(95%CI)分别为0.77(0.65-0.90)、0.59(0.40-0.88)、0.39(0.18-0.86)。结论冷冻单囊胚移植周期中,胚胎气液滴位置距宫底的距离1cm以下时,妊娠率和活产率最高。Objective:To investigate the association between air bubbles location and live birth rate after single frozen-thawed blastocyst transfer.Methods:A retrospective study was performed in 3077 single frozen-thawed blastocyst transfer cycles between 1st January,2012 to 31th December,2017.After the embryos were deposited in the uterus,the distance from the transfer air bubble to the fundus,as seen on abdominal ultrasound,was measured.According to the air bubbles distance,the cases were categorized into four groups:D1:<1 cm(n=2016),D2:1-1.5 cm(n=902),D3:1.51-1.9cm(n=124),D4:>1.9 cm(n=35).The clinical outcomes were compared between groups.Results:From Group D1 to Group D4,the pregnancy rate was 55.5%,48.8%,42.7% and 31.4%,respectively,and the live birth rate was 46.5%,40.6%,34.7% and 25.7%,respectively,showing a gradually decreasing trend(P<0.01).Adjusted for confounders,the odds ratio(95%CI)for pregnancy comparing Group D2,D3 and D4 with Group D1 was 0.74(0.63-0.87),0.58(0.4-0.84)and 0.35(0.17-0.72),respectively.The odds ratio(95%CI)for live birth comparing Group D2,D3 and D4 with Group D1 was 0.77(0.65-0.90),0.59(0.40-0.88)and 0.39(0.18-0.86),respectively.Conclusion:In the frozen single blastocyst transplantation cycle,the pregnancy rate and live birth rate are highest when the distance from air bubbles locations to fundus was less than 1cm.
