摘 要:目的:分析利奈唑胺治疗革兰阳性球菌感染重症肺炎的疗效及其对血清炎症因子的影响。方法:选取2017年4月~2019年5月在本院呼吸与危重症医学科住院治疗的130例革兰阳性球菌感染重症肺炎患者,采用随机数字表法分为观察组(n=65)和对照组(n=65)。观察组患者应用利奈唑胺注射液(600mg/次,1次/12 h);对照组患者应用去甲万古霉素(0.8 g/次,1次/12 h)。治疗2周后比较两组患者的总有效率、临床症状改善时间以及不良反应发生率,同时比较两组患者治疗前后的转化生长因子-β(transforming growth factor-β,TGF-β)、C反应蛋白(C-reaction protein,CRP)、白介素-1 (interleukin-1,IL-1)和白介素-2(interleukin-2,IL-2)水平。结果:治疗2周后,观察组总有效率(90.769%)高于对照组(67.692%,P <0.05);观察组的临床症状改善时间均短于对照组(P <0.05);两组患者不良反应的发生率比较无统计学差异(P>0.05);观察组和对照组治疗前TGF-β、CRP、IL-1和IL-2水平比较无统计学差异(P>0.05);治疗后观察组的TGF-β水平高于对照组,其CRP、IL-1和IL-2水平低于对照组(P <0.05)。结论:利奈唑胺治疗革兰阳性球菌感染重症肺炎有较好疗效,可有效改善其症状,缓解炎症反应。Objective:To analyze the efficacy of linazolamide in the treatment of severe pneumonia infected with Gram-positive cocci and its effect on serum inflammatory factors.Methods:From 2017 April to May 2019,130 patients with severe pneumonia infected with gram-positive coccus admitted in the department of respiratory and critical care medicine of Zhengzhou people's hospital were selected and divided randomly into observation group(n=65)and the control group(n=65).Patients in the observation group were treated with rina thiazole amine(600 mg/times,1 time/12 h),while pathents in the control group were treated with vancomycin(0.8 g/time,1 time/12 h).After 2 weeks of treatment,the total effective rate and the clinical symptoms improved time,rate of adverse reactions were compared.Meanwhile,the levels of transforming growth factor-type(TGF-β),C-reaction protein(CRP),interleukin-1(IL-1)and interleukin-2(IL-2)in the two groups were compared.Results:After 2 weeks of treatment,the total effective rate of in the observation group was 90.769%,which was higher than that in the control group of 67.692%,with statistical significance(P<0.05).The improvement time of clinical symptoms in the observation group was significantly shorter than that in the control group(P<0.05).The difference of the rate of adverse reactions between the two groups showed no significant difference(P>0.05).Before treatment,there was no significant difference in TGF-β,CRP,IL-1 and IL-2 levels in the observation group and the control group(P>0.05).After treatment,obvious higher TGF-βlevel and lower levels of CRP,IL-1 and IL-2 were found in the observation group than in the control group(P<0.05).Conclusion:Linezolid is effective in the treatment of severe pneumonia with Gram-positive cocci infection,which can effectively improve the symptoms and relieve the inflammatory reaction.
