RhD阴性育龄妇女中RHD1227A基因型检测的临床意义

摘　　要：目的:研究RhD阴性育龄妇女RHD1227A基因型检测的价值。方法:采用血清学方法检测RhD血型,采用分子生物学技术(熔解曲线分析法)检测RHD1227A基因。结果:261例RhD初筛阴性妇女,RHD1227A基因型46例,占17.62%;且RHD1227A基因型只存在于93例含C抗原的表现型中,占这类表现型的49.46%。对38例生育1胎的RHD1227A基因型妇女进行生育2胎意愿调查,检测前后,有意愿从43.75%提升到87.5%。结论:RhD阴性育龄妇女RHD1227A基因型是常见基因型,与含C抗原表现型密切相关,对其检测有利于制定胎-母免疫保护策略和提高育龄妇女2胎意愿。Objective:To assess the potential value of RHD1227 A detection in RhD antigen negative women. Method:Serology and molecular biology techniques(Dissociation Curve method)were used to detected RhD blood group and RHD1227 A gene. Result:Among the 261 RhD negative individuals,46 cases(17.62%)were found as RHD1227 A.In addition,we found 93 individuals were positive for C antigen phenotype.Interestingly,all the RHD1227 A identified individuals were C antigen positive(49.46%).Screening 38 RHD1227 A genotypic individuals,we found a dramatic increase on their desire to have a second child from before RHD1227 A detection(43.75%)to after RHD1227 A detection(87.5%).Conclusion:RHD1227 A is a common genotype among RhD negative women at child-bearing age.It is strongly associated with C antigen phenotypes.RHD1227 A determination may have beneficial effects on strategies for fetal-material immune protection from alloimmunization and strengthen their desire to have a second child.
