糖化血红蛋白、糖化白蛋白、糖化白蛋白/糖化血红蛋白检测在妊娠期糖尿病(GDM)早期诊断中的价值

摘　　要：目的探讨糖化血红蛋白(HbAlc)、糖化白蛋白(GA)、糖化白蛋白/糖化血红蛋白检测在妊娠期糖尿病(gestational diabetes mellitus,GDM)早期筛查中的价值。方法妊娠24~28周的孕妇218例,进行OGTT检测,根据IADPSG新诊断标准,区分出GDM组和OGTT正常组,并同时测定这两组孕妇的HbAlc和GA、OGTT水平,计算GA/HbAlc比值。结果 (1) GDM组HbAlc、GA、FPG、1hPG和2h PG水平、GA/HbAlc比值均显著高于OGTT正常组(P<0.05)。(2) Pearson相关性比较显示,两组中GA与HbAlc均具有相关性,r值分别是0.932和0.378,而GA与FPG和1hPG、GA/HbAlc比较只有GDM组具有相关性,r值分别是0.854、0.389和0.888。(3)两组GA、HbAlc、GA/HbAlc比值阳性检出率比较,GDM组GA/HbAlc阳性率高于HbAlc、GA组(P<0.05)。结论 GDM患者中,GA/HbAlc、HbAlc、GA是GDM的诊断因子,与低GA/HbAlc相比高GA/HbAlc患者GDM发生率更高,在GDM患者中,早期检测GA/HbAl水平有助于综合治疗。Objective To explore the value of HbAlc, GA, and GA/HbAlc detection in the early screening of gestational diabetes mellitus(GDM). Methods 218 pregnant women between 24-28 weeks of gestation were tested for OGTT. According to the new diagnostic criteria of IADPSG, the GDM group and the OGTT normal group were distinguished, and the HbAlc, GA and OGTT levels of these two groups of pregnant women were measured at the same time, and the GA/HbAlc ratio was calculated. Results(1) The levels of HbAlc, GA, FPG,1 hPG and 2 hPG, and the ratio of GA/HbAlc in the GDM group were significantly higher than those in the OGTT normal group(P<0.05).(2) Pearson correlation comparison shows that GA and HbAlc are correlated in the two groups, and the values are 0.932 and 0.378, respectively. Compared with GA and FPG, 1 hPG and GA/HbAlc, only the GDM group is correlated. The r values are respectively.0.854, 0.389 and 0.888.(3) In comparison of the positive detection rate of GA, HbAlc, and GA/HbAlc ratio between the two groups, the positive rate of GA/HbAlc in the GDM group was higher than that in the HbAlc and GA groups(P<0.05). Conclusion In GDM patients, GA/HbAlc, HbAlc, and GA are the diagnostic factors of GDM. Compared with low GA/HbAlc,the incidence of GDM is higher in patients with high GA/HbAlc. In GDM patients, early detection of GA/HbAl levels is conductive to comprehensive treatment.
