摘 要:目的探讨矫正年龄1岁前不同程度支气管肺发育不良(BPD)患儿潮气肺功能临床变化特点。方法选取2017年1月—2019年10月于青岛大学附属医院住院治疗的BPD早产儿73例为观察组,根据BPD严重程度分为Ⅰ级41例(A组),Ⅱ、Ⅲ级32例(B组),并选取同期住院且胎龄、体重相当的非BPD早产儿45例为对照组(C组),比较3组患儿矫正1、6、12月龄时潮气肺功能参数包括达峰时间比(tPF%tE)、达峰容积比(VPF%VE)、每分通气量(MV)、呼吸频率(RR)、潮气量(VT)、吸呼比(TI/TE)及25%、50%、75%潮气量时呼气流速(TEF25、TEF50、TEF75)的差异。结果矫正年龄、组别均对患儿潮气呼吸肺功能参数tPF%tE、VPF%VE、RR、TEF50、TEF25有显著影响(F矫正年龄=28.61~164.05,F组别=8.01~14.30,P<0.05),矫正年龄与组别间无交互作用;3组患儿tPF%tE、VPF%VE、VT、TEF75、TEF50、TEF25、MV参数在矫正6、12月时较矫正1月龄时水平明显升高,RR明显下降,差异具有统计学意义(F矫正年龄=28.61~280.45,P<0.05);矫正1、6月龄时,A组和B组tPF%tE、VPF%VE、TEF50、TEF25参数明显低于C组,RR高于C组,差异具有统计学意义(F=3.54~11.39,P<0.05);矫正12月龄时,仅B组tPF%tE、VPF%VE、TEF50、TEF25参数低于C组,RR高于C组,差异具有统计学意义(F=2.80~4.58,P<0.05)。结论BPD患儿矫正年龄1岁内潮气肺功能存在阻塞性改变,且BPD程度越重,阻塞越明显。Objective To investigate clinical changes in tidal lung function in children with different degrees of broncho-pulmonary dysplasia(BPD)within 1 year of corrected age.Methods We selected 73 preterm infants with BPD who were hospitalized in the The Affiliated Hospital of Qingdao University between January 2017 to October 2019,and divided them,according to the severity of BPD,into group A(gradeⅠ,41 cases)and group B(gradeⅡorⅢ,32 cases).Forty-five non-BPD preterm infants were selected as the control(group C)who were hospitalized during the same time and had comparable gestational age and weight.Tidal lung function parameters were compared between the infants of the three groups at 1,6,and 12 months of corrected age,including the ratio of time to peak tidal expiratory flow to total expiratory time(tPF%tE),the ratio of volume to peak tidal expiratory flow to total expiratory volume(VPF%VE),minute ventilation(MV),respiratory rate(RR),tidal volume(VT),inspiratory-to-expiratory time ratio(TI/TE),and expiratory flow rate when 25%,50%,and 75%of tidal volume remains in the lungs(TEF25,TEF50,and TEF75).Results There were significant differences in the tidal breathing parameters tPF%tE,VPF%VE,RR,TEF50,and TEF25 between different corrected ages and groups(Fage=28.61-164.05,Fgroup=8.01-14.30,P<0.05).There was no interaction between corrected age and group.All the three groups had significantly higher tPF%tE,VPF%VE,VT,TEF75,TEF50,TEF25,and MV and significantly lower RR at the corrected age of 6 months and 12 months than at 1 month(Fage=28.61-280.45,P<0.05).At the corrected age of 1 month and 6 months,group A and group B showed significantly lower tPF%tE,VPF%VE,TEF50,and TEF25 than group C,RR was higher than group C(F=3.54-11.39,P<0.05).At the corrected age of 12 months,tPF%tE,VPF%VE,TEF50,and TEF25 in group B were significantly lower than those in group C,RR was higher than group C(F=2.80-4.58,P<0.05).Conclusion There are obstructive changes in tidal lung function in infants with BPD within 1 year of corrected age.The more
