瓜蒌薤白半夏汤对不稳定型心绞痛患者MMP-9及TIMP-1的影响

摘　　要：目的:观察瓜蒌薤白半夏汤对不稳定型心绞痛(unstable angina,UA)痰浊闭阻证患者的临床疗效及对基质金属蛋白酶-9(matrix metalloproteinase,MMP-9)、基质金属蛋白酶抑制剂-1(tissue inhibitor of matrix metalloproteinases,TIMP-1)的影响。方法:将64例UA痰浊闭阻证患者随机分为对照组和治疗组各32例。对照组使用西医常规治疗,治疗组在对照组治疗的基础上采用瓜蒌薤白半夏汤治疗,两组均连续治疗2周。观察两组患者治疗前后的中医证候疗效、MMP-9及TIMP-1变化情况。结果:对照组有效率为59.4%,治疗组有效率为84.4%,两组有效率比较,差异有统计学意义(P<0.05)。治疗后两组患者心绞痛发作次数、持续时间、硝酸甘油消耗量较治疗前均明显下降(P<0.05),组间比较,差异有统计学意义(P<0.05)。治疗后两组患者血浆MMP-9水平明显下降(P<0.01),TIMP-1水平下降不明显(P>0.05)。组间比较,MMP-9水平差异具有统计学意义(P<0.05),TIMP-1水平差异无统计学意义(P>0.05)。两组患者血浆MMP-9治疗前后差值比较,差异有统计学意义(P<0.01);两组患者血浆TIMP-1治疗前后差值比较,差异无统计学意义(P>0.05)。结论:瓜蒌薤白半夏汤联合西药治疗可改善UA患者的临床症状,降低MMP-9水平。Objective:To observe the influence of Trichosanthes and Chinese Chive and Pinellia Decoction on matrix metalloproteinase-9(MMP-9)and tissue inhibitor of matrix metalloproteinases(TIMP-1)of unstable angina(UA)with syndrome of phlegm and turbidity retention.Methods:A total of 64 patients of UA with syndrome of phlegm and turbidity retention were randomly divided into the control group and the treatment group,with 32 cases in each group.The control group was treated with conventional Western medicine,while the treatment group was treated with Trichosanthes and Chinese Chive and Pinellia Decoction on the basis of the control group.Both groups were treated for 2 weeks.The curative effect of TCM syndrome,the changes of MMP-9 and TIMP-1 in the two groups were observed before and after the treatment.Results:The effective rate was 59.4%in the control group and 84.4%in the treatment group,and the difference was statistically significant(P<0.05).After the treatment,angina attack frequency,duration and nitroglycerin consumption of the two groups significantly decreased compared with those before the treatment(P<0.05),and the differences between the two groups were statistically significant(P<0.05).After the treatment,the plasma MMP-9 level in the two groups decreased significantly(P<0.01),but the level of TIMP-1 did not decrease significantly(P>0.05).The difference of MMP-9 level between the two groups was statistically significant(P<0.05),but the difference of TIMP-1 level was not statistically significant(P>0.05).The difference of plasma MMP-9 between the two groups before and after the treatment was statistically significant(P<0.01),while that of plasma TIMP-1 between the two groups before and after the treatment was not statistically significant(P>0.05).Conclusion:Trichosanthes and Chinese Chive and Pinellia Decoction combined with western medicine can improve the clinical symptoms of UA patients and reduce the level of MMP-9 of patients.
