中西医结合治疗溃疡性结肠炎研究进展

摘　　要：溃疡性结肠炎的西医治疗方法包括药物治疗、非药物治疗和手术治疗,其中药物治疗包括氨基水杨酸类药物、糖皮质激素、免疫抑制剂、生物制剂、微生态制剂;非药物治疗包括粪菌移植、干细胞移植和高压氧疗法。溃疡性结肠炎的中医治疗方法包括辨证论治、分期论治、专方专药、中药灌肠、针灸治疗等。近年来,关于溃疡性结肠炎的病因及发病机制的探讨越来越深入,但对溃疡性结肠炎的药理学机制和作用靶点仍有待挖掘。中医的证候研究尚缺乏大样本、多中心的数据比对。如大部分研究采用有效率和缓解率作为疗效判定标准,对复发和不良事件的发生缺少长时间的追踪和关注。临床治疗中,患者症状可得以缓解,但黏膜愈合和组织学缓解较缓慢,如何优化治疗方案,需进一步探索研究。Western medicine treatments of ulcerative colitis include drug therapy,non-drug therapy and surgical therapy.Drug therapy includes aminosalicylic acid drugs,glucocorticoids,immunosuppressants,biological agents and microecological agents.Non-drug therapy includes fecal bacteria transplantation,stem cell transplantation and hyperbaric oxygen treatment.The TCM therapies of ulcerative colitis include treatment based on syndrome differentiation,treatment by stages,special formula and medicine,enema of TCM,acupuncture and so on.In recent years,the research on etiology and pathogenesis of ulcerative colitis deeper and deeper,but the pharmacological mechanism and targets of ulcerative colitis still need to be explored and updated.TCM syndrome research is still lack of large sample,multi-center data comparison.For example,most studies use the effective rate and remission rate as the criteria of curative effect,and lack of long-term follow-up and attention on recurrence and adverse events.In clinical practice,the symptoms of patients can be relieved,but the mucosal healing and histological remission are relatively slow.Therefore,how to optimize the treatment plan needs further exploration and research.
