加味人参养荣汤治疗维持性血透肾性贫血的疗效观察及对铁调素、纤维蛋白降解产物和免疫功能的影响

摘　　要：目的:探讨加味人参养荣汤治疗维持性血透肾性贫血患者的临床疗效以及对患者铁调素、纤维蛋白降解产物(FDP)和免疫功能的影响。方法:60例维持性血液透析患者随机分为对照组和观察组,对照组进行常规促红细胞生成素(EPO)等治疗;观察组在对照组治疗基础上服用加味人参养荣汤治疗。两组在治疗前、治疗后检测外周血血红蛋白(Hb)、红细胞压积(HCT)、血清铁调素、血清铁、铁蛋白、FDP和CD_(4)^(+)、CD_(8)^(+)、CD_(4)^(+)/CD_(8)^(+)等指标。结果:治疗后观察组临床疗效优于对照组;观察组Hb、HCT升高较对照组显著(P<0.05);两组铁调素水平均降低,血清铁和铁蛋白升高,且观察组较对照组变化显著(P<0.05);对照组患者的FDP水平与治疗前无显著差异(P>0.05),观察组FDP水平则较治疗前显著降低(P<0.05);两组CD_(8)^(+)在治疗前后无明显变化(P>0.05),两组治疗后CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)均有提高,且观察组较对照组显著(P<0.05)。结论:加味人参养荣汤可调节维持性血透肾性贫血患者铁调素和FDP水平,改善贫血状态,提高免疫功能。Objective:To investigate the clinical efficacy of Jiawei Renshen Yangrong Decoction in the treatment of patients with maintenance hemodialysis anemia and the effects on hepcidin,fibrin degradation products(FDP)and immune function.Methods:60 Cases with maintenance hemodialysis anemia were randomly divided into a control group and an observation group.The control group was treated with conventional erythropoietin(EPO)and other treatments.The observation group was treated with Jiawei Renshen Yangrong Decoction on the basis of the treatment of the control group.The peripheral hemoglobin(Hb),hematocrit(HCT),serum hepcidin,serum iron,ferritin,FDP and CD_(4)^(+),CD_(8)^(+),CD_(4)^(+)/CD_(8)^(+)indexes were measured before and after treatment.Results:The clinical effect of the observation group was better than that of the control group after treatment(P<0.05);Hb and HCT in the observation group were significantly higher than those in the control group(P<0.05);serum iron and ferritin of the two groups were increased,and that in observation group were significantly more significant than those of the control group(P<0.05);the FDP level of the control group was not significantly different from before treatment(P>0.05),but the FDP level of the observation group was lowered significantly.CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+)of the two groups were raised,the changes in observation group were more significant(P<0.05),there was no obvious changes in_CD(8)^(+).Conclusion:Jiawei Renshen Yangrong Decoction can regulate the levels of hepcidin and FDP in patients with maintenance hemodialysis anemia,improve the state of anemia,improve immune function.
