腰椎退行性疾病患者行内固定配合椎间植骨融合术后CT影像学效果评价

摘　　要：目的分析CT影像学对腰椎退行性疾病患者行内固定配合椎间植骨融合术后的效果。方法选取2016年6月到2018年7月我院骨科收治的行椎间植骨融合内固定术患者50例。记录患者术前以及术后1个月、6个月、1年腰椎正侧位CT检查结果,观察随访期间椎间隙高度、有效上终板面积、腰椎术后融合率的CT影像学变化。结果术后不同时间椎间隙高度与术前相比有显著改善(P<0.05),术后6个月和术后1年植骨量体积为5、6mL患者较植骨量体积4mL患者椎间隙高度升高明显(P<0.05);术后6个月、1年随访发现,椎间植骨量体积5、6mL患者植骨融合率显著高于椎间植骨量体积4mL患者(P<0.05)。结论内固定配合椎间植骨融合术是治疗腰椎退行性疾病的有效手术方案,以5mL及以上植骨量体积进行自体骨植骨融合术,显著提高术后椎间融合成功率,CT影像学检查对评估内固定配合椎间植骨融合术治疗效果具有可靠价值。Objective To analyze the effect of CT imaging on lumbar degenerative diseases after internal fixation combined with interbody fusion.Methods From June 2016 to July 2018,50 patients with lumbar degenerative changes were treated with interbody fusion and internal fixation.Anteroposterior and lateral lumbar CT examination were recorded before the operation and 1 month,6 months and 1 year after the operation.The height of intervertebral space,effective upper endplate area and fusion rate of the lumbar spine were observed du ring the follow-up period.Results The intervertebral space height was significantly improved at different times after operation(P<0.05).The intervertebral space height of patients with 5 mL and 6 mL bone graft volume at 6 months and 1 year after operation was significantly higher than that in patients with 4 mL bone graft volume(P<0.05);the volume of intervertebral bone graft volume was found after 6 months and 1 year of follow-up The fusion rate of 5 mL and 6 mL patients was significantly higher than that of 4 mL intervertebral bone graft volume(P<0.05).Conclusion Internal fixation combined with interbody fusion is an effective surgical scheme for treating of lumbar degenerative diseases.Autogenous bone graft fusion with 5 mL or more bone graft volume can significantly improve the success rate of postoperative intervertebral fusion.CT imaging examination has reliable value in evaluating the therapeutic effect of internal fixation combined with interbodyfusion.
