摘 要:目的研究奥塔戈运动锻炼(OEP)在颅脑损伤患者术后康复中的应用效果。方法选择2016年7月至2019年6月我院收治的颅脑损伤患者94例,按随机数字表法分为试验组与对照组各47例。对照组行常规康复干预,试验组行OEP。观察两组康复前后肢体运动功能、维持平衡情况、跌倒效能、生活自理能力,血清铁蛋白(SF)、神经元特异性烯醇化酶(NSE)等神经功能指标,大脑中动脉舒张末期峰值流速(Vd)、收缩末期峰值流速(Vp)等大脑血流动力学指标。结果康复后,试验组简易运动功能量表(FMA)、Berg平衡量表(BBS)、跌倒效能量表(MFES)及Barthel评分均大于对照组(P<0.05),血清NSE、SF水平低于对照组,大脑中动脉Vd、Vp均高于对照组(P<0.05)。结论OEP应用于颅脑损伤术后康复,可改善患者大脑血流动力学及神经功能,提高其平衡能力、跌倒效能、肌力及生活自理能力。Objective To investigate the clinical efficacy of Otago exercise program(OEP)for postoperative rehabilitation of patients with brain injury.Methods A total of 94 brain injury patients in our hospital from July 2016 to June 2019 were involved in this study.They were divided into experimental or control group by using the random number table method,47 in each group.The control group was given conventional rehabilitation while the experimental group was given OEP.The limb motor function,maintain balance,falls performance,life self-care ability,neurological function indexes such as serum ferritin(SF)and neuron-specific enolase(NSE),and cerebral hemodynamics indexes such as middle cerebral artery end diastolic velocity(Vd)and middle cerebral artery peak systolic velocity(Vb)were observed before and after rehabilitation.Results After rehabilitation,the FMA scores,BBS scores,MFES scores and Barthel scores in the experimental group were higher than those in the control group(P<0.05).The levels of serum SF and NSE in the experimental group were lower than those in the control group(P<0.05).The middle cerebral artery Vd and Vb in the experimental group were higher than those in the control group(P<0.05).Conclusion Application of OEP in postoperative rehabilitation of brain injury can improve the cerebral hemodynamics and neurological function of patients,and improve their balance ability,falls performance,muscle strength and life self-care ability.
