血清hCG和孕酮的检测对异位妊娠早期诊断的价值研究

摘　　要：目的探讨血清hCG、孕酮的检测在异位妊娠早期诊断中的重要价值。方法选取2019年6月至2020年9月在我院就医并确诊为子宫外妊娠的96例患者为研究组,正常子宫内妊娠的100例受试者为对照组。分别对两组人群进行血清hCG、孕酮的测定,比较两组检测结果的差异;计算工作特征曲线(ROC曲线)的曲线下面积(AUC),确定诊断分界点,对血清hCG、孕酮的诊断效能进行评价。结果研究组和对照组的血清hCG、孕酮检测结果进行比较,两者均存在差异且差异具有统计学意义(P<0.05)。研究组血清hCG、孕酮的检测数值明显低于对照组的数值。观察血清hCG、孕酮的ROC曲线的AUC数值,发现在异位妊娠的早期诊断中,血清hCG、孕酮的检测具有良好的诊断价值。其中血清hCG的AUC达到0.92,诊断分界点为14458 IU/L,灵敏度为80.7%,特异性为87.9%。孕酮的AUC达到0.9,诊断分界点为14.36 ng/mL,灵敏度为83.9%,特异性为83.3%。结论血清hCG、孕酮的检测在异位妊娠早期正确诊断上具有良好的价值,可以为临床提高诊断效能。Objective To investigate the important value of serum hCG and progesterone in the early diagnosis of ectopic pregnancy.Methods From June,2019 to September,2020,96 patients with ectopic pregnancy were selected as the study group,while 100 patients with normal intrauterine pregnancy were selected as the control group.Serum hCG and progesterone were measured in both groups,and the differences between the two groups were evaluated.The area under the curve(AUC)value of the working characteristic curve(ROC curve)was calculated to determine the diagnostic cut-off point and the diagnostic efficacy of serum hCG and progesterone.Results The serum hCG and progesterone test results of the study group and the control group were compared,and the differences were statistically significant(P<0.05).Specifically,the values of hCG and progesterone in the ectopic pregnancy group were significantly lower than those in the normal intrauterine pregnancy group.When calculating the AUC value of serum hCG and progesterone,it was found that the detection of serum hCG and progesterone had a good diagnostic value in the early diagnosis of extrauterine pregnancy.The AUC of serum hCG was 0.92,with the cut-off point 14458 IU/L,the sensitivity 80.7%,and the specificity 87.9%.The AUC of progesterone was 0.9,with the cut-off point 14.36 ng/mL,the sensitivity 83.9%,and the specificity 83.3%.Conclusion The detection of serum hCG and progesterone has a good value in the early diagnosis of ectopic pregnancy,which can improve the diagnostic efficiency for clinical practices.
