未成年人血浆PT、INR、APTT、FIB参考区间的初步建立

摘　　要：目的建立适用于SYSMEX CS-5100全自动血凝分析仪及Siemens公司凝血酶原时间(PT)、国际标准化比值(INR)、活化部分凝血酶时间(APTT)、纤维蛋白原(FIB)试剂的未成年人参考区间。方法收集2018年4月至2018年9月就诊于同仁医院眼科未成年患者2218例,成年患者460例,使用SYSMEX CS-5100全自动血凝分析仪及Siemens公司的相关凝血试剂进行检测,初步建立本实验室未成年人参考区间。结果未成年人群不同性别组间凝血4项检测结果差异无统计学意义(P>0.05)。PT/INR在未成年人群各组间差异无统计学意义,参考区间为PT 10.91~13.37s,INR 0.90~1.13;APTT(FSL)在婴儿期、幼儿期及学龄前期及学龄期四组间检测结果差异无统计学意义(P>0.05),即12岁以下未成年人参考区间为24.30~40.48s,青春期参考区间为22.40~36.74s;FIB参考区间婴儿组1.02~3.42g/L,幼儿组1.21~3.43g/L,其他年龄组间检测结果差异无统计学意义(P>0.05),即3岁以上未成年人1.35~3.54g/L。结论初步建立本实验室凝血酶原时间(PT)、国际标准化比值(INR)、活化部分凝血酶时间(APTT)、纤维蛋白原(FIB)的未成年人参考区间,为临床提供可靠诊疗依据。Objective To establish a reference range for Sysmex cs-5100 automatic blood coagulation analyzer,including prothrombin time(PT),international normalized ratio(INR),activated partial thromboplastin time(APTT)and fibrinogen(FIB)reagents of Siemens company.Methods 2218 cases of juvenile patients and 460 cases of adult patients in Tongren Hospital from April,2018 to September,2018 were collected and detected by Sysmex cs-5100 automatic blood coagulation analyzer and relevant coagulation reagents of Siemens company,and the reference interval of minor population in our laboratory was then initially established.Results There was no significant difference in coagulation test results among different gender groups of minors(P>0.05).The reference interval of PT was 10.91-13.37 seconds,and that of INR was 0.90-1.13.There was no significant difference in the results of APTT(FSL)among infancy,infancy,preschool and school age groups(P>0.05).The reference interval of children under 12 years old was 24.30-40.48 seconds,that of puberty was 22.40-36.74 seconds.FIB reference range was 1.02-3.42 g/L in the infant group and 1.21-3.43 seconds in the children group.There was no significant difference among other age groups(P>0.05),which was 1.35-3.54g/l for minors over 3 years old.Conclusion The reference intervals of prothrombin time(PT),international normalized ratio(INR),activated partial thromboplastin time(APTT)and fibrinogen(FIB)in our laboratory are preliminarily established to provide a reliable basis for clinical diagnosis and treatment.
