摘 要:目的分析桥本氏甲状腺炎(HT)与HLA-DQA1、DQB1、DRB1基因多态性的相关性。方法以测序分型法(PCR-SBT)法对152例HT患者和176例健康对照人群HLA-DQ和HLA-DR基因进行分型分析,观察HT患者与健康对照人群的等位基因分布。结果HT患者中DQA1*0302、DQB1*0601、DRB1*04、DRB1*14等位基因频率明显高于对照人群,而DQA1*0102、DQB1*0602、DRB1*13、单倍体DQA1*01-DQB1*06-DRB1*15(DR15)明显低于对照人群。结论HLA-DQA1*0302、DQB1*0601、DRB1*04、DRB1*14与HT的易感性相关,而HLA-DQA1*0102、DQB1*0602、DRB1*13、DR15与HT的保护性基因相关,HLA-DQ、DR基因位点存在保护性和易感性的双重作用,其等位基因之间的共同作用是影响HT发病的重要遗传因素。Objective To analyze the relationship between Hashimoto′s thyroiditis(HT)and HLA-DQA1,DQB1,DRB1 gene polymorphisms.Methods HLA-DQ and HLA-DR genes were analyzed by PCR-SBT among 152 HT patients and 176 healthy controls.Results The allele frequencies of DQA1*0302,DQB1*0601,DRB1*04 and DRB1*14 in HT patients were significantly higher than those in controls,while DQA1*0102,DQB1*0602,DRB1*13,haploid DQA1*01-DQB1*06-DRB1*15(DR15)were significantly lower in HT patients than in controls.Conclusion HLA-DQA1*0302,DQB1*0601,DRB1*04,DRB1*14 could be associated with the predisposing effects to HT,while HLA-DQA1*0102,DQB1*0602,DRB1*13 and DR15 could be related to the protective effect to HT.HLA-DQ and DR loci have dual effects of both protection and predisposition and the interaction of HLA-DQ and DR alleles is an important genetic factor affecting the pathogenesis of HT.
