丙肝抗体联合丙肝核心抗原在丙型肝炎诊断中的作用

摘　　要：目的分析丙肝抗体与丙肝核心抗原联合诊断丙型肝炎的临床效果。方法选取180例2017年1月—2018年6月在本单位采集的单试剂丙型肝炎阳性的献血者血样,对血清标本的丙肝抗体、丙肝核心抗原进行检测,分析不同方法诊断丙型肝炎的效果。结果以重组免疫印迹试验结果为诊断标准,丙肝抗体诊断丙型肝炎的灵敏度、特异度、阳性预测值、阴性预测值分别为84.52%、94.79%、93.42%、87.50%;丙肝核心抗原诊断结果分别为77.38%、96.88%、95.59%、83.04%;联合诊断结果为96.43%、95.83%、95.29%、96.84%,联合检测的灵敏度、阴性预测值均高于单独指标检测(P<0.05)。结论丙肝抗体与丙肝核心抗原联合检测,可有效提升丙型肝炎筛查效果。Objective To analyze the clinical effect of hepatitis C antibody combined with hepatitis C core antigen in the diagnosis of hepatitis C.Methods 180 blood samples were collected from single reagent hepatitis C positive donors in our unit from January 2017 to June 2018.The serum samples were tested for hepatitis C antibody and core antigen of hepatitis C,and the effects of different methods in the diagnosis of hepatitis C were analyzed.Results Used the results of the recombinant western blot test as the diagnostic criteria,the sensitivity,specificity,positive predictive value,and negative predictive value of hepatitis C antibodies in the diagnosis of hepatitis C were 84.52%,94.79%,93.42%and 87.50%respectively;the diagnostic results of hepatitis C core antigen were 77.38%,96.88%,95.59%,83.04%respectively;combined diagnosis results were 96.43%,95.83%,95.29%,96.84%respectively.The sensitivity and negative predictive value of combined detection were higher than those of single index detection(P<0.05).Conclusion The combined detection of hepatitis C antibody and hepatitis C core antigen can effectively improve the screening effect of hepatitis C.
