摘　　要：目的:探讨筋膜鞘悬吊睑板结合提上睑肌缩短术(LMR)治疗重度上睑下垂效果。方法:回顾性分析35例(40只眼)采用筋膜鞘悬吊睑板结合LMR治疗的重度上睑下垂患者和31例(37只眼)采用LMR治疗的重度上睑下垂患者的临床资料,分别记为A组和B组。对比两组术后1个月、3个月上睑回退量和上睑活动范围;对比两组术前、术后1个月、3个月眼表功能指标;对比两组矫治效果、患者美容满意度及术后3个月内并发症发生率。结果:两组术后3个月上睑回退量均减少,且A组少于B组;两组术后3个月上睑活动范围均增加,且A组高于B组,差异均有统计学意义(P<0.05)。A组矫治效果优良率和美容总满意率分别为100.00%和97.14%,均高于B组的86.49%和77.42%,差异有统计学意义(P<0.05)。两组SIt、BUT组间、时间、交互作用比较差异均无统计学意义(P>0.05)。A组术后1个月FL高于术前,术后3个月FL低于术后1个月,B组术后1个月和术后3个月FL均高于术前,且术后1个月和术后3个月A组FL均低于B组,差异均有统计学意义(P<0.05)。两组并发症发生率比较差异无统计学意义(P>0.05)。结论:筋膜鞘悬吊睑板结合LMR治疗重度上睑下垂术后恢复快,效果好,美容作用强,术后眼表功能恢复迅速,且安全。Objective To explore the effect of conjoint fascial sheath suspension meibomian plate combined with levator muscle resection(LMR)in the treatment of severe blepharoptosis.Methods The clinical data of 35 patients(40 eyes)with severe blepharoptosis treated by conjoint fascial sheath suspension meibomian plate combined with LMR and 31 patients(37 eyes)with severe blepharoptosis treated by LMR were analyzed retrospectively,who were recorded as group A and group B respectively.The amount of upper eyelid retraction and the range of upper eyelid movement were compared between the two groups at 1 month and 3 months after operation.The ocular surface function indexes of the two groups were compared before operation,at 1 month and 3 months after operation.The treatment effect,cosmetic satisfaction and the incidence of complications within 3 months were compared between the two groups.Results Three months after operation,the amount of upper eyelid regression decreased in both groups,and the amount in group A was less than that in group B;the range of upper eyelid movement increased in both groups,and the amount in group A was higher than that in group B,the differences were statistically significant(P<0.05).The excellent and good rate of group A and the total satisfaction rate of cosmetic were 100.00%and 97.14%respectively,which were higher than those of group B(86.49%and 77.42%,P<0.05).There were no significant differences in SIT and BUT between the two groups(P>0.05).FL at one month after operation in group A was higher than that before operation,and FL at 3 months after operation was lower than that at one month after operation(P<0.05).FL at one month and 3 months after operation in group B was higher than those before operation(P<0.05).And FL at one month and 3 months after operation in group A was lower than those in group B(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion The conjoint fascial sheath suspension meibomian plate combined with LMR in s
