颞浅动脉额支皮瓣修复颧颞部皮肤软组织缺损的疗效探讨

摘　　要：目的:探讨颞浅动脉额支皮瓣在颧颞面部皮肤软组织缺损修复中的临床疗效。方法:选取2016年6月-2019年10月笔者医院收治的颧颞部皮肤肿瘤患者10例。切除病灶,术中送病理检查明确性质,确保恶性病灶切除干净无残留。采用多普勒血流探测仪测定并标出颞浅动脉额支走行,根据受区缺损形态和功能设计皮瓣,进行皮瓣移植修复。结果:本组10例患者皮瓣均完全成活,供区直接缝合,切口甲级愈合。术后随访3~18个月,患者对颧颞部皮瓣外形及功能满意,鬓角再造2例。额部供区无瘢痕,毛发遮挡看不见切口痕迹,恶性肿瘤均无复发。结论:浅动脉额支皮瓣修复颧颞肿瘤术后皮肤软组织缺损具有良好效果,皮瓣成活率高,组织应用代价小,供区切口愈合,患者对外观及功能满意。Objective To investigate the curative effect of superficial temporal artery frontal branch skin flap for repairing zygomatic-temporal skin defect.Methods 10 patients with zygomatic-temporal skin tumors admitted to our hospital from June 2016 to October 2019 were selected.The lesions were removed,the pathological examination was sent to confirm the nature during the operation,and ensure that the malignant lesions were removed without residue.The doppler blood flow detector was used to measure and mark the frontal branch of the superficial temporal artery.The flap was designed according to the shape and function of the defect in the affected area,and the flap was designed and repaired.Results 10 patients with zygomatic-temporal skin tumors in this group,all the flaps survived completely,the donor area was sutured directly,and the incision healed in grade A.The patients were followed up for 3 to 18 months,the patient was satisfied with the shape and function of the zygomatictemporal flap,and the temples were reconstructed in 2 cases.There was no scar in the donor area of the forehead,no trace of incision can be seen when the hair was covered,and there was no recurrence of malignant tumors.Conclusion The superficial temporal artery frontal branch skin flap has a good effect on repairing skin and soft tissue defects after zygomatic-temporal tumors operation.The flap has a high survival rate,low cost of tissue application,and the donor site incision heals.The patient was satisfied with the appearance and function.
