浆细胞性乳腺炎的超声影像特征分析

摘　　要：目的:探讨浆细胞性乳腺炎的超声影像特征。方法:回顾性分析大同市第三人民医院2016年9月至2019年9月治疗的浆细胞性乳腺炎患者80例的临床及超声影像资料,比较患者实质外形特征及团块型边界特征,以病理诊断为标准,分析超声检查误诊情况。结果:浆细胞性乳腺炎,80例患者共发现椭圆形实质21例,不规则形实质46例,圆盘形实质10例,类圆形实质3例,不同实质外形特征均差异有统计学意义(χ^(2)=6.978、8.670、3.521、3.143,均P<0.05)。80例浆细胞性乳腺炎患者超声影像诊断共误诊4例,误诊率为5.00%(4/80),其中1例1型单纯导管扩张型,2例2型囊肿型,1例3型团块型。结论:超声影像诊断浆细胞性乳腺炎能获得明确的病灶实质外形特征及团块型边界特征,且误诊率较低,有助于临床医师进行早期诊断。Objective To investigate the ultrasonographic features of plasma cell mastitis.Methods The clinical and ultrasonic imaging data of 80 patients with plasma cell mastitis who received treatment from September 2016 to September 2019 in the Third People's Hospital of Datong were retrospectively analyzed.The parenchyma appearance characteristics and mass boundary characteristics were compared.The misdiagnosis of ultrasound examination was analyzed based on the pathological diagnosis.Results Among 80 patients with plasma cell mastitis,21 had oval parenchyma,46 had irregular parenchyma,10 had disc-shaped parenchyma,and 3 had round parenchyma.There was significant difference in shape characteristics of parenchyma(χ^(2)=6.978,8.670,3.521,3.143,all P<0.05).The misdiagnosis rate was 5.00%(4/80).Among four misdiagnosed patients,1 patient had type 1 simple ductal dilatation,2 patients had type 2 cysts and 1 case had type 3 mass.Conclusion Ultrasound diagnosis of plasma cell mastitis can obtain clear parenchyma features and mass boundary features,with low misdiagnosis rate,which is helpful for clinicians to make early diagnosis.
