丙种球蛋白联合激素治疗小儿腺病毒肺炎临床疗效观察

摘　　要：目的探讨丙种球蛋白联合激素治疗小儿腺病毒肺炎(AP)疗效观察及对免疫功能和炎症因子的影响。方法选择2019年1月至2020年6月于杭州市儿童医院就诊的AP患儿62例为研究对象,按不同治疗方法分为观察组(丙种球蛋白联合甲泼尼龙)31例和对照组(甲泼尼龙)31例,疗程均为5~7天,比较两组治疗后疗效、主要症状和体征消失时间、免疫功能及炎症因子变化。结果观察组总有效率高于对照组(96.78%vs.74.19%,χ^(2)=4.679,P<0.05);观察组AP患儿咳嗽明显缓解、退热和肺部湿啰音消失时间均短于对照组(t值分别为12.409、10.435、7.375,均P<0.05);两组AP患儿治疗后CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)较治疗前均升高,且治疗后观察组高于对照组(t值分别为7.330、7.549、4.769,均P<0.05);两组AP患儿治疗7天血清降钙素原(PCT)、C-反应蛋白(CRP)较治疗前均降低,且观察组低于对照组(t值分别为14.782、6.509,均P<0.05)。结论丙种球蛋白联合激素治疗小儿AP疗效良好,且可增强患儿免疫功能,减轻患儿炎症反应。Objective To investigate the therapeutic effect of gamma globulin combined with hormone in the treatment of adenovirus pneumonia(AP)for children and its effects on immune function and inflammatory factors.Methods 62 children with AP who were treated in Hangzhou Children’s Hospital from January 2019 to June 2020 were selected as the research objects.According to different treatment methods,all of them were divided into observation group(gamma globulin combined with methylprednisolone)and control group(methylprednisolone)with 31 cases in each group,the course of treatment was 5~7 days.The curative effects,disappearance time of main symptoms and signs,immune function and inflammatory factors after treatment were compared between the two groups.Results The total effective rate of observation group was higher than that of control group(96.78%vs.74.19%,χ^(2)=4.679,P<0.05).The durations of cough and fever,and the time of lung moist rales disappeared in the observation group were shorter than those in control group(t values were 12.409,10.435 and 7.375,respectively,all P<0.05).After treatment,the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)for AP children in both two groups were higher than before treatment,and in observation group after treatment was higher than those in control group(t values were 7.330,7.549 and 4.769,respectively,all P<0.05).Serum procalcitonin(PCT)and C-reactive protein(CRP)of the two groups of AP children in 7 days after treatment were both lower than that before treatment,and in the observation group was lower than the control group(t values were 14.782 and 6.509,respectively,all P<0.05).Conclusion Gamma globulin combined with hormone has a good effect on AP in children,which can enhance the immune function of children and reduce their inflammatory reaction.
