摘　　要：根据温州地区地处沿海、以湿为主的气候特点,笔者阐述了该地区新型冠状病毒肺炎的中医病因病机、辨证分型和治法方药,并分析病案1则。该地区新型冠状病毒肺炎可归属于中医学疫病、湿疫范畴,病位在肺,可伤及心、肝、胃、肠,主要病机为寒、热、湿、毒四者错杂,但化热伤津为其根本,当早期投大剂量清热解毒之品,以断其热势,不令传变。初期证属湿毒蕴肺者,方可选藿香正气散合银翘散加减。中期证属疫毒闭肺者,方可选麻杏石甘汤合小陷胸汤、苇茎汤加减;证属气血两燔者,方可选清瘟败毒散加减。危重症期证属内闭外脱者,方可选参附汤或生脉散加减。恢复期证属肺脾气虚者,方可选参苓白术散合补肺汤加减。因人、因地制宜,通过辨证论治,以中西医结合治疗新型冠状病毒肺炎,疗效显著。Based on the coastal location and humid climate of Wenzhou,the authors expounded the etiology,pathogenesis,syndrome differentiation typing,principles,methods,prescriptions and medicine of Corona Virus Disease2019(COVID-19)from the perspective of Chinese medicine,and analyzed one case.The COVID-19 in this area can be classified as a disease of pestilence and a damp epidemic in Chinese medicine.The location of it is in the lung,and it damages the heart,liver,stomach and intestines.The main pathogenesis of COVID-19 is mixed cold,heat,damp and toxin,but its fundamental is the transformation into heat and the damage to fluid.In early stage,a large dose of heatclearing and detoxifying Chinese herbs should be applied to terminate its tendency of heat and transmutation.For patients with syndrome of dampness-toxin in the lung in early stage,modified Huoxiang Zhengqi powder combined with Yinqiao powder should be prescribed.For patients with syndrome of epidemic toxin obstructing the lung in middle stage,modified Maxing Shigan tang combined with Xiaoxianxiong tang and Weijing tang should be applied.For patients with syndrome of dual blaze of qi-blood aspects in middle stage,modified Qingwen Baidu powder should be used.For critically ill patients with syndrome of internal block and external collapse,modified Shenfu tang or Shengmai powder should be adopted.For patients with syndrome of dual deficiency of the lung-spleen in convalescence stage,modified Shenling Baizhu powder and Bufei tang should be employed.In conclusion,the integrated Chinese and western medicine therapy has significant therapeutic effect in treating therapy COVID-19 based on the considerations of person and place in diagnosis and treatment,and through syndrome differentiation and treatment.
