补肾通痹泰颗粒联合麝香海马追风膏治疗膝骨关节炎临床研究  

Clinical Study on Bushen Tongbitai Granules Combined with Shexiang Haima Zhuifeng Plaster for Knee Osteoarthritis

摘　　要：目的:观察补肾通痹泰颗粒联合麝香海马追风膏治疗膝骨关节炎的临床疗效。方法:将膝骨关节炎患者100例按随机数字表法分为2组,对照组50例采用补肾通痹泰颗粒治疗;观察组50例在对照组的基础上联合麝香海马追风膏外敷治疗。比较2组治疗前后疼痛视觉模拟评分法(VAS)评分、膝关节功能评分量表(Lysholm)评分、膝关节西安大略和麦克马斯特大学骨关节炎指数量表(WOMAC)评分及炎症因子白细胞介素-1β(IL-1β)、一氧化氮(NO)、白细胞介素-1(IL-1)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)的变化情况。结果:治疗后,2组VAS评分均较治疗前明显降低(P<0.05),Lysholm评分较治疗前明显升高(P<0.05);且观察组2项评分改善较对照组更显著(P<0.05)。治疗后,2组关节日常功能、疼痛、僵硬评分及总评分均较治疗前明显降低(P<0.05),且观察组各项评分均低于对照组(P<0.05)。治疗后,观察组血清IL-1β水平较治疗前明显升高(P<0.05),血清NO、IL-1、IL-6、TNF-α水平明显降低(P<0.05);且观察组各项指标改善较对照组更显著(P<0.05)。结论:补肾通痹泰颗粒联合麝香海马追风膏治疗膝骨关节炎能缓解关节炎症反应,改善关节功能,减轻疼痛,疗效优于单用通痹泰颗粒治疗。Objective:To observe the clinical effect of the therapy of Bushen Tongbitai granules combined with Shexiang Haima Zhuifeng plaster for knee osteoarthritis.Methods:A hundred cases with knee osteoarthritis were divided into two groups according to the random number table method,50 cases in each group.The control group was treated with Bushen Tongbitai granules;the observation group was additionally treated with external application of Shexiang Haima Zhuifeng plaster based on the treatment of the control group.Before and after treatment,the scores of Visual Analogue Scale on pain(VAS),Lysholm Knee Scale(Lysholm)and Western Ontario and McMaster University Osteoarthritis Index(WOMAC),and the levels of inflammatory factor of interleukin-1β(IL-1β),nitric oxide(NO),interleukin-1(IL-1),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were compared between the two groups.Results:After treatment,the VAS scores in both groups were significantly decreased when compared with those before treatment(P<0.05),and the scores of Lysholm were significantly increased(P<0.05);the improvement of the above two scores was more significant in the observation group than in the control group(P<0.05).After treatment,the scores of daily function,pain,stiffness and the total scores in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the above scores in the observation group were lower than those in the control group(P<0.05).After treatment,the level of IL-1βin serum in the observation group was significantly increased when compared with that before treatment(P<0.05),and the levels of NO,IL-1,IL-6 and TNF-αwere significantly decreased when compared with those before treatment(P<0.05);the changes of the above indexes in the observation group were more significant than those in the control group(P<0.05).Conclusion:The therapy of Bushen Tongbitai granules combined with Shexiang Haima Zhuifeng plaster for knee osteoarthritis,can relieve inflammatory reactions,improve joint function and relie
