前列地尔结合丹参注射液治疗老年髋部骨折下肢深静脉血栓护理研究

摘　　要：目的:观察老年髋部骨折下肢深静脉血栓(DVT)采用前列地尔结合丹参注射液治疗的临床价值和护理效果。方法:将老年髋部骨折患者84例根据数字表法随机分为对照组和观察组,每组42例。2组实施手术后,对照组给予低分子肝素钠皮下注射,观察组给予前列地尔注射液联合丹参注射液静脉滴注,连续治疗2周。对比2组DVT发生率和48 h伤口引流量、治疗前后髌下10 cm周径、视觉模拟评分法(VAS)评分、血小板、活化部分凝血酶原时间(APTT)、D-二聚体(D-D)的差异。结果:观察组DVT发生率较对照组低,48 h伤口引流量较对照组少,差异有统计学意义(P<0.05)。2组治疗前髌下10 cm周径、VAS评分比较,差异无统计学意义(P>0.05)。2组治疗后髌下10 cm周径、VAS评分均改善,观察组治疗后髌下10 cm周径较对照组短,VAS评分较对照组低,差异有统计学意义(P<0.05)。2组治疗前血小板、APTT、D-D比较,差异无统计学意义(P>0.05)。2组治疗后血小板、APTT、D-D较前改善,观察组治疗后血小板计数较对照组少,APTT较对照组短,D-D较对照组低,差异有统计学意义(P<0.05)。结论:老年髋部骨折下肢DVT采用前列地尔和丹参注射液联合治疗,能明显降低下肢DVT发生率,减轻患者疼痛症状。Objective:To observe the clinical value and nursing effect of alprostadil combined with salvia injection for lower extremity deep venous thrombosis(DVT)in senile patients with hip fracture.Methods:A total of 84 cases of senile patients with hip fracture were divided into the control group and the observation group according to the random number table method,42 cases in each group.After operation,the control group was given subcutaneous injection of low molecular weight heparin for two weeks,and the observation group was given intravenous infusion of alprostadil injection combined with salvia injection for two weeks.The differences in DVT incidences,48-hour wound drainage,10 cm infrapatellar circumferences before and after treatment,Visual Analogue Scale(VAS)scores,platelets,activated partial thromboplastin time(APTT),and D-dimer(D-D)between the two groups were compared.Results:The incidence of DVT in the observation group was lower than that in the control group,and the 48-hour wound drainage was less,the difference being significant(P<0.05).Before treatment,there was no significant difference being found in the comparison of the 10 cm infrapatellar circumferences and VAS scores between the two groups(P>0.05).After treatment,the 10 cm infrapatellar circumferences and VAS scores in the two groups were improved when compared with those before treatment;the 10 cm infrapatellar circumference in the observation group was shorter than that in the control group,and the VAS score was lower,the difference being significant(P<0.05).Before treatment,there was no significant difference being found in the comparison of platelets,APTT and D-D between the two groups(P>0.05).After treatment,platelets,APTT and D-D in the two groups were improved when compared with those before treatment;the platelet count in the observation group was fewer than that in the control group,APTT was shorter,and D-D was lower,the difference being significant(P<0.05).Conclusion:The application of alprostadil combined with salvia injection for DVT in se
