摘 要:目的:探讨阿昔洛韦治疗EB病毒感染引起的传染性单核细胞增多症的应用疗效。方法:从本院收治的传染性单核细胞增多症患者中选取62例作为观察对象,病例收治时间为2018年4月-2020年4月,按照抽选法随机分成两组。对照组(n=31)常规内科药物治疗,观察组(n=31)在对照组基础上增加阿昔洛韦治疗,比较两组患者的治疗效果、症状指标改善、T淋巴细胞水平变化。结果:观察组、对照组治疗总有效率分别为96.77%(30/31)、74.19%(23/31),对比差异有统计学意义(P<0.05)。观察组患者颈部淋巴结肿大消退时间、退热时间、咽痛恢复时间等症状改善时间均早于对照组患者,对比差异有统计学意义(P<0.05)。两组患者治疗前CD3+、CD4+、CD8+、CD4+/CD8+水平对比差异无统计学意义(P>0.05);治疗后观察组患者CD3+、CD8+水平低于对照组,CD4+、CD4+/CD8+高于对照组(P<0.05)。观察组患儿EBV-DNA转阴率均高于对照组,差异有统计学意义(P<0.05)。结论:应用阿昔洛韦治疗传染性单核细胞增多症的临床显著,使患者的T淋巴细胞水平控制在稳定区间,减少咽部排毒,减轻呼吸道梗阻症状,加快血EBV-DNA转阴,以获得更为理想的疗效。Objective:To investigate the therapeutic effect of Acyclovir on infectious mononucleosis caused by EB virus infection.Method:A total of 62 patients with infectious mononucleosis in our hospital were selected as the observation objects.The patients were admitted from April 2018 to April 2020,and were randomly divided into two groups according to the method of selection.The control group(n=31)received conventional medical treatment,and the observation group(n=31)added Acyclovir treatment on the basis of the control group.The treatment efficacy,symptom index improvement and T lymphocyte level changes of the two groups were compared.Result:The total effective rates of the observation group and the control group were 96.77%(30/31)and 74.19%(23/31),respectively,with significant difference(P<0.05).The improvement time of cervical lymph node enlargement and regression time,fever reduction time and sore throat recovery time in the observation group were erlier than those in the control group,with statistical significance(P<0.05).Before treatment,the levels of CD3+,CD4+,CD8+,CD4+/CD8+in the two groups had no difference(P>0.05);after treatment,the levels of CD3+,CD8+in the observation group were lower than those in the control group,while the levels of CD4+,CD4+/CD8+in the observation group were higher than those in the control group(P<0.05).The negative rate of EBV-DNA in the observation group was higher than that in the control group(P<0.05).Conclusion:Acyclovir has significant clinical effect in the treatment of infectious mononucleosis.It can control the level of T lymphocyte in the stable range,reduce the throat detoxification,alleviate the symptoms of respiratory tract obstruction,and accelerate the negative EBV-DNA in blood,so as to obtain more ideal curative effect.
