锁骨钩钢板固定与单纯同种异体肌腱重建喙锁韧带治疗肩锁关节脱位临床效果比较

摘　　要：目的:分析锁骨钩钢板固定与单纯同种异体肌腱重建喙锁韧带治疗肩锁关节脱位临床效果。方法:选择2016年7月-2019年6月本院收治的80例肩锁关节脱位患者为研究对象。将其随机分为两组,每组40例。对照组患者接受单纯钩钢板治疗。观察组患者接受同种异体肌腱重建喙锁韧带联合锁骨钩钢板治疗。结果:观察组患者住院时间及手术时间明显更短,出血量明显更少,术后VAS评分更低,ASES分数更高,喙锁间距测定值小于对照组(P<0.05)。结论:肩锁关节脱位患者采用自体或同种异体肌腱重建喙锁韧带联合锁骨钩钢板进行治疗,能够取得满意效果。此类方法有助于提升患者术后肩关节功能,疼痛感小,安全性强,有效性高。值得进一步在临床中推广应用。Objective:To analyze the clinical effect of clavicular hook plate fixation and coracoclavicular ligament reconstruction with allogenic tendon alone in the treatment of acromioclavicular dislocation.Method:A total of 80 patients with acromioclavicular dislocation admitted to our hospital from July 2016 to June 2019 were selected as the study subjects.They were randomly divided into two groups with 40 cases in each group.The control group were treated with hook plate alone.The observation group received autologous or allogeneic tendon reconstruction of coracoclavicular ligament combined with clavicular sulcus plate for treatment of disease.Result:The hospitalization time and operation time of the observation group were significantly shorter,the amount of bleeding was significantly less,the postoperative VAS score was lower,the ASES score was higher,and measured value of the coracoclavicular distance was less than that of the control group (P<0.05).Conclusion:For patients with acromioclavicular dislocation,coracoclavicular ligament reconstruction with allogenic tendon alone combined with clavicular sulcus plate can achieve satisfactory results.Such methods help to improve the shoulder function of patients after surgery,with less pain,strong safety and high effectiveness.Therefore,it is worthy of further promotion and application in clinical practice.
