超声引导下脱细胞异体真皮基质治疗高位肛瘘的临床研究

摘　　要：目的:探究高位肛瘘患者在超声引导下运用脱细胞异体真皮基质治疗的临床效果。方法:选取本院2017年5月-2020年5月入院的肛瘘患者120例,按照随机分组法分为观察组和对照组,每组60例。对照组予肛瘘挂线术治疗,观察组在超声引导下运用脱细胞异体真皮基质填塞治疗。对两组患者临床治疗有效率、术后肛瘘复发率及术后患者疼痛评分、创面分泌物量评分、创面愈合时间情况进行比较。结果:观察组患者治疗有效率为96.7%,对照组为91.7%(P>0.05);两组患者术后肛瘘复发率比较,差异无统计学意义(P>0.05);观察组术后疼痛及创面分泌物量评分明显优于对照组(P<0.05);观察组术后创面愈合时间显著短于对照组(P<0.05)。结论:高位肛瘘患者在超声引导下运用脱细胞异体真皮基质治疗的临床效果显著,能有效缓解患者疼痛症状,减少术后创面分泌物量,且缩短患者术后创面愈合时间,促进患者疾病恢复,值得推广应用。Objective:To explore the clinical efficacy of acellular allogenic dermal matrix under the guidance of ultrasound in patients with high anal fistula.Method:A total of 120 patients with high anal fistula admitted in our hospital from May 2017 to May 2020 were divided into observation group and control group according to the randomization method.The treatment group was treated by acellular allogenic dermal matrix tamponade under ultrasound guidance.The effective rate of clinical treatment,anal fistula recurrence rate and postoperative pain score,wound secretion volume,wound healing time were compared between the two groups.Result:The effective rate of the treatment group was 96.7% in the observation group and 91.7% in the control group (P>0.05).The recurrence rate of the anal fistula in the two groups was comparable (P>0.05);the postoperative pain and wound secretion (P<0.05).The wound healing time in the observation group was significantly lower than that in the control group (P<0.05).Conclusion:The use of acellular allogenic dermal matrix in the treatment of high anal fistula can effectively relieve pain symptoms,reduce postoperative wound secretion,shorten the wound healing time and promote the recovery of patients’ disease.This treatment is worth spreading and applying.
