摘　　要：目的:评估抑酸治疗对成人合并有咽喉反流患者扁桃体切除术后疼痛的影响,为临床疼痛管理提供指导。方法:选择2017年6月-2019年12月85例需行扁桃体切除术并通过反流症状指数量表及反流体征指数量表确定为合并咽喉反流的患者,随机分为A、B、C三组,A组术前1周开始至术后14 d持续使用抑酸治疗,B组在术后14 d内使用抑酸治疗,C组全程不给予抑酸治疗。在术后1、5、9、14 d应用疼痛视觉模拟量表评分(VAS)法评估疼痛情况。结果:所有患者术后均有疼痛症状,术后第1天三组患者疼痛症状差异无统计学意义(P>0.05);术后第5、9、14天A组、B组疼痛评分均小于C组,且在第5、9天,A组也小于B组(P<0.05);但A、B两组14 d疼痛评分相似,差异无统计学意义(P>0.05)。结论:抑酸治疗能有效缓解合并咽喉反流患者扁桃体切除术后疼痛症状,术前1周用药效果更佳。Objective:To evaluate the effect of acid inhibition therapy on pain after tonsillectomy in adults with pharyngeal reflux and to provide guidance for clinical pain management.Method:A total of 85 patients who underwent tonsillectomy from June 2017 to December 2019 were selected and identified as pharyngeal reflux by reflux symptom index(RSI)and reflux finding score(RFS),and randomly divided into three groups,group A was treated with acid inhibition from one week before operation to 14 days after operation,group B was treated with acid inhibition within 14 days after operation,and group C was not treated with acid inhibition during the whole course.Pain was evaluated by visual analogue scale(VAS)at 1,5,9 and 14 days after operation.Result:All patients had pain symptoms after operation.The pain symptoms of the three groups were similar on the first day after operation(P>0.05).On the 5th,9th and 14th day after operation,the pain score in group A and B was lower than group C,and on the 5th and 9th day,the score in group A were also lower than group B(P<0.05).However,on the 14th day,the pain scores of patients in group A and group B were similar(P<0.05).Conclusion:Acid inhibition therapy can effectively relieve the pain symptoms after tonsillectomy in patients with pharyngeal reflux,and the effect is better one week before operation.
