尿沉渣染色镜检联合显色培养法快速鉴定尿路感染酵母样真菌的应用价值的探讨

摘　　要：目的探讨尿沉渣染色镜检联合显色培养法对尿液标本中酵母样真菌的快速诊断价值。方法取尿培养标本共376例,以常规尿培养法的结果为金标准,评估尿沉渣染色镜检法联合显色培养法对真菌(以下简称联合法)的检出效果。结果(1)常规尿培养中酵母样真菌阳性率为9.0%(34/376);联合法酵母样真菌的阳性率为6.9%(26/376)。(2)经配对卡方检验,联合法与常规尿培养法在检出酵母样真菌方面的差异有统计学意义,在鉴定酵母样真菌上一致性较高(Kappa=0.830,P<0.05);在常规尿培养法检出的34例酵母样真菌标本中,镜检联合显色培养法检出26例,符合率为76.4%(26/34),联合法比常规培养法至少提前一天鉴定出酵母样真菌。结论尿沉渣染色镜检联合显色培养法与常规尿培养法对酵母样真菌的检出符合率较高,且能显著缩短检测时限,可作为常规尿培养法的补充实验。Objective To evaluate the performance of microscopy of stained urinary sediment in combination with a chromogenic culture method(combined method)for the detection of yeast-like fungi in urine specimens.Methods Routine culture test was used as golden standard,for the performance evaluation of the combined method.Results(1)Yeast-like fungi were observed in 34,among the 376 specimens,by routine culture method(34/376,9%);whereas 26 specimen were observed fungus-positive by combined mthod(26/376,6.9%).(2)As for the performance,significant difference was observed between the combined method and the routine culture method(p=0.000),but the two methods were consistent with each other at a high degree(Kappa=0.83,P<0.05);of all the 34 yeast-like fungus positive specimen by routine culture method,26 were observe positive by combined method(76.4%,26/34),which could give a fungal species information 1 or 2 days earlier than the routine culture method.Conclusion Due to its higher consistency,obviously shorted test period and availability,relative to routine culture method,the combined method can be a complementary way for the earlier detection of yeast-like fungi.
