痰热清注射液对痰浊壅肺型AECOPD机械治疗患者肺功能与血气分析指标的影响

摘　　要：目的:探讨痰热清注射液对痰浊壅肺型慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)机械治疗患者肺功能与血气分析指标的影响。方法:将痰浊壅肺型AECOPD机械治疗患者144例采用随机数字表法分为两组各72例。对照组采用常规治疗,研究组在对照组基础上加用痰热清注射液。观察患者临床疗效、肺功能指标和血气分析指标变化情况及不良反应发生率。结果:研究组总有效率为94.4%(68/72),高于对照组的73.6%(53/72)(P<0.05)。研究组患者咳嗽、咳痰消失时间和住院时间短于对照组(P<0.05)。治疗后两组肺功能指标较治疗前升高,且研究组高于对照组(P<0.05)。治疗后两组pH值较治疗前无明显改变(P>0.05),两组动脉血氧分压(partial pressure of oxygen,PaO2)较治疗前升高(P<0.05),动脉二氧化碳分压(partial pressure of carbon dioxide,PaCO2)较治疗前降低(P<0.05),且研究组PaO2高于对照组(P<0.05),PaCO2低于对照组(P<0.05)。两组药物不良反应发生率比较差异无统计学意义(P>0.05)。结论:痰热清注射液对痰浊壅肺型AECOPD机械治疗患者的辅助治疗有助于改善患者肺功能与血气分析指标。Objective:To explore the effects of Tanreqing injection on pulmonary function and blood gas analysis in AECOPD patients of phlegm-dampness in the lung pattern who accepted mechanical treatment.Methods:All 144 patients were allocated to two groups according to random number table method,72 cases each group.The control group adopted conventional therapy,and the study group adopted Tanreqing injection on the foundation of the therapy the control group accepted.To observe clinical effects,the indexes of pulmonary function and blood gas analysis of the patients,as well as the incidence of adverse reactions.Results:Total effective rate of the study group was 94.4%(68/72),higher than 73.6%(53/72)of the control group(P<0.05).The study group was shorter than the control group in cough disappearing time,expectoration disappearing time and hospitalization time(P<0.05).After treating,the indexes of pulmonary functions in both groups rose compared with before treating,and the rise of the study group was superior to that of the control group(P<0.05).pH values showed no obvious changes in both groups after treating than before treating(P>0.05),PaO2 rose than before treating in both groups(P<0.05),PaCO2 lowered than before treating(P<0.05),PaO2 of the study group was higher than that of the control group(P<0.05),PaCO2 lower than that of the control group(P<0.05).The difference had no statistical meaning in the incidence of adverse reaction between both groups(P>0.05).Conclusion:Tanreqing injection could help improve the indexes of pulmonary function and blood gas analysis.
