JCI标准下跌倒风险分级护理在老年住院患者防跌倒管理中的作用

摘　　要：目的:探讨基于国际医疗卫生机构认证联合委员会(joint commission international,JCI)标准下跌倒风险分级护理在老年科住院患者护理中的有效性。方法:以2015年1月至2016年12月的29 581例年龄>60岁的住院患者作为对照组,行常规跌倒预防护理。以2017年1月至2018年10月的35 005例年龄>60岁的住院患者作为观察组,观察组在常规跌倒预防护理基础上,行JCI标准下的跌倒风险分级护理。对两组患者跌倒事件发生率及对护理的满意率进行比较。结果 :经评估,观察组中低危患者18 207例(52.01%),中危患者11 968例(34.19%),高危患者4830例(13.80%)。观察组患者跌倒事件发生率为0.06%(22/35 005),低于对照组的0.11%(32/29 581),差异有统计学意义(P<0.05)。观察组中曾跌倒患者对护理工作满意度为90.91%(20/22),对照组中曾跌倒患者对护理工作满意度为65.63%(21/32),差异有统计学意义(P<0.05)。结论:对老年住院患者实施基于JCI标准下跌倒风险分级护理,能够降低老年患者跌倒的发生率,提高患者的满意度,是一个有效的防跌倒风险的方法。Objective: To explore the effectiveness of grading nursing of fall risk in the care for the inpatients in the department of geriatrics based on JCI standard. Methods: All 29 581 inpatients elder than 60 years from January, 2015 to December, 2016 were chosen as the control group, and they accepted conventional fall prevention care. 35 005 inpatients elder than 60 years from January, 2017 to October, 2018 as the observation group and they received grading nursing of fall risk under JCI standard based on conventional fall prevention care.To compare the incidences of fall and nursing satisfactory rate between both groups. Results: Through evaluating,in the observation group, there were 18 207 medium and low risk patients(52.01%), 11 968 medium risk patients(34.19%) and 4830 high risk patients(13.80%). The incidence of fall in the observation group was 0.06%(22/35 005),lower than 0.11%(32/29581) of the control group, and the difference was statistically significant(P<0.05).Satisfaction of patients with fall to nursing work in the observation group was 90.91%(20/22), higher than 65.63%(21/32) of the control group, and the difference had statistical meaning(P<0.05). Conclusion: The implementation of grading nursing of fall risk under JCI standard for the elderly patients could lower the incidence of fall in the elderly patients and raise the patients’ satisfaction, and it is an effective method of fall risk prevention.
