摘  要:目的探讨多序列磁共振成像(MRI)联合磁共振波谱(MRS)诊断低级别脑胶质瘤的临床价值。方法回顾性抽取2017年1月至2019年12月广西医科大学第二附属医院收治的脑胶质瘤患者47例,术前均行多序列MRI、MRS影像学检查。47例患者由术后病理确诊低级别、高级别脑胶质瘤,统计其影像学诊断符合率,分析MRS扫描相关指标值。结果 47例患者术后病理诊断低级别脑胶质瘤18例,高级别脑胶质瘤29例。多序列MRI联合MRS对脑胶质瘤高级别、低级别的诊断符合率高于单项多序列MRI(χ^(2)=5.600、6.026,P=0.018、0.014)。MRS扫描结果显示,低级别与高级别脑胶质瘤肿瘤实质区胆碱复合物与N-乙酰天门冬氨酸比值、胆碱复合物与肌酸比值高于瘤周水肿区(P<0.05),且相应值高级别脑胶质瘤高于低级别(P<0.05)。结论多序列MRI联合MRS可有效鉴别低级别脑胶质瘤。Objective To investigate the value of multiple sequence magnetic resonance imaging(MRI)combined with magnetic resonance spectroscopy(MRS)in the diagnosis of low-grade gliomas.Methods A retrospective sample of 47 patients with glioma who were admitted to the Second Affiliated Hospital of Guangxi Medical University from January 2017 to December 2019,all of them underwent multiple sequence MRI and MRS imaging examinations before surgery.Forty-seven patients were diagnosed with low-grade and high-grade gliomas by postoperative pathology.The coincidence rate of imaging diagnosis was counted and the relevant index values of MRS scan were analyzed.Results Forty-seven patients were pathologically diagnosed with 18 low-grade gliomas and 29 high-grade gliomas.Among the patients,the coincidence rate of multi-sequence MRI combined with MRS for low-grade gliomas were higher than that of single multi-sequence MRI(χ^(2)=5.600,6.026,P=0.018,0.014).The results of MRS scan showed that the ratio of choline complex to N-acetylaspartate and the ratio of choline complex to creatine in the parenchymal area of low-grade and high-grade glioma were higher than those in the peritumoral edema area(P<0.05),and which of high-grade glioma were higher than those of low-grade(P<0.05).Conclusions Multiple sequence MRI combined with MRS can effectively differentiate low-grade gliomas.
