摘  要:目的考察CD_(4)^(+)T细胞miR-29家族和干扰素γ(Interferon-γ,IFN-γ)在痰菌阴性肺结核诊断中的临床价值。方法随机选取2017年12月-2018年12月济南市人民医院感染性疾病科收治的痰菌阴性肺结核患者50例,痰菌阳性肺结核患者50例,健康体检者50名作为对照组。磁性活化细胞分选法(MACS)分选CD_(4)^(+)细胞。酚氯仿抽提总RNA。室温聚合酶链式反应(RT-PCR)法测定各组患者miR-29a、miR-29b、miR-29c和IFN-γmRNA表达水平。ELISA法测定血清白细胞介素(Interleukin-6,IL-6)、IL-10、IL-23和骨桥蛋白。结果痰菌阴性组miR-29b、miR-29c分别为2.57±0.82、2.65±0.92高于对照组(P<0.001),痰菌阴性组miR-29a为0.48±0.09低于对照组(P<0.001);痰菌阴性组IFN-γmRNA为0.15±0.06低于对照组(P<0.001)。痰菌阴性组IL-6、IL-10、IL-23分别为(50.15±17.85)、(26.48±5.79)、(99.45±16.48)pg/ml高于对照组(P<0.05)。骨桥蛋白为(699.45±316.81)pg/ml高于对照组(P<0.001)。结论痰菌阴性肺结核高表达miR-29b和miR-29c mRNA,低表达miR-29a和IFN-γmRNA,临床上可将CD4+T细胞miR-29家族及IFN-γ基因用于痰菌阴性肺结核的检测。OBJECTIVE To investigated the clinical value of mir-29 family and interferon-γ of CD_(4)^(+) T cell in the diagnosis of sputum negative pulmonary tuberculosis. METHODS From Dec. 2017 to Dec. 2018, 50 cases of sputum-negative tuberculosis, 50 cases of sputum-positive tuberculosis who were admitted to the Department of Infectious Diseases of Jinan People′s Hospital were randomly selected, and 50 cases of health subjects were selected as the control group. CD_(4)^(+) cells were sorted by magnetic activated cell sorting(MACS) methods. Total RNA was extracted by phenol chloroform. Room temperature synthase chain reaction(RT-SCR) method was used to determine the expression levels of miR-29 a, miR-29 b, miR-29 c and IFN-γ mRNA in each group. ELISA method was used to determine serum IL-6, IL-10, IL-23 and osteopontin. RESULTS The miR-29 b and miR-29 c in sputum negative group were 2.57±0.82 and 2.65±0.92 respectively, which were significantly higher than those in control group(P<0.001). The miR-29 a in sputum negative group was 0.48±0.09, significantly lower than that in control group(P<0.001);the IFN-γ mRNA in sputum negative group was 0.15±0.06, significantly lower than that in control group(P<0.001). The levels of IL-6, IL-10 and IL-23 in sputum negative group were(50.15±17.85) pg/ml,(26.48±5.79) pg/ml and(99.45±16.48) pg/ml, respectively, which were significantly higher than those in control group(P<0.05). Osteopontin was(699.45±316.81) pg/ml, significantly higher than that of control group(P<0.001). CONCLUSION The expression of miR-29 b,miR-29 c and mRNA were increased in sputum-negative pulmonary tuberculosis patients, and the expression of miR-29 a and IFN-γ were decreased. Clinically, the miR-29 family and IFN-γ genes of CD4+ T cells can be used to detect sputum-negative tuberculosis.
