摘  要:目的探讨多囊卵巢综合征合并感染患者病原菌特征及对血清细胞因子、磷脂酰肌醇3-激酶(PI3K)/丝氨酸-苏氨酸蛋白激酶(AKT)信号转导通路蛋白水平的影响。方法选取2016年3月-2019年10月遵义医科大学附属医院多囊卵巢综合征合并感染患者96例为感染组,同期多囊卵巢综合征未合并感染患者96例为未感染组。统计感染组患者病原菌分布特征、感染组不同感染类型(革兰阴性菌、革兰阳性菌)患者血清细胞因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)、IL-6]、PI3K、AKT蛋白水平,并绘制受试者工作特征(ROC)曲线及曲线下面积(AUC)探讨血清细胞因子(TNF-α、IL-8、IL-6)、PI3K、AKT蛋白水平单一、联合检测对患者革兰阴性菌感染类型的鉴别价值。结果感染组96例患者共分离出病原菌96株,其中革兰阴性菌59株占61.46%,革兰阳性菌37株占38.54%;感染组血清TNF-α、IL-8、IL-6与PI3K、AKT蛋白分别为(147.48±34.19)ng/ml、(224.72±46.34)ng/ml、(75.02±16.51)ng/ml与(0.81±0.19)、(0.88±0.21)高于未感染组(P<0.001);革兰阴性菌感染患者血清TNF-α、IL-8、IL-6与PI3K、AKT蛋白分别为(168.07±37.26)ng/ml、(253.41±49.28)ng/ml、(86.75±18.37)ng/ml与(0.94±0.22)、(1.03±0.24)高于革兰阳性菌感染患者(P<0.05);鉴别革兰阴性菌的AUC:TNF-α为0.701,IL-8为0.719,IL-6为0.808,PI3K蛋白为0.763,AKT蛋白为0.791,各指标联合为0.894,联合诊断优于单一诊断(P<0.05)。结论多囊卵巢综合征合并感染患者病原菌以革兰阴性菌为主,可引起TNF-α、IL-8、IL-6及PI3K/AKT信号通路的高表达,TNF-α、IL-8、IL-6、PI3K/AKT联合检测有助于鉴别革兰阴性和阳性菌。OBJECTIVE To investigate the characteristics of pathogenic bacteria in patients with polycystic ovary syndrome complicated by infection and their effects on serum cytokines, phosphatidylinositol 3-kinase(PI3 K)/serine-threonine protein kinase(AKT) signal transduction pathway protein levels. METHODS From Mar. 2016 to Oct. 2019, 96 patients with polycystic ovary syndrome and infection in the Affiliated Hospital of Zunyi Medical University were selected as the infection group, and 96 patients with polycystic ovary syndrome without infection during the same period were selected as the uninfected group. The distribution characteristics of the pathogens in the infection group, serum cytokines [tumor necrosis factor-α(TNF-α), interleukin-8(IL-8), interleukin-6(IL-6)], PI3 K, AKT protein levels in patients with different infection types(Gram-negative bacteria, Gram-positive bacteria) in the infection group were statistically analyzed, the receiver operating characteristic(ROC) curve and the area under the curve(AUC) were drawn to explore the value of single, combined detection of serum cytokines(TNF-α, IL-8, IL-6), PI3 K, and AKT proteins in the identification of the types of gram-negative bacteria infections in patients. RESULTS A total of 96 pathogenic bacteria were isolated from 96 patients in the infection group, of which 59 gram-negative bacteria accounted for 61.46%, and 37 gram-positive bacteria accounted for 38.54%. Serum TNF-α, IL-8, IL-6 and PI3 K and AKT proteins in the infection group were(147.48±34.19) ng/ml,(224.72±46.34) ng/ml,(75.02±16.51) ng/ml and(0.81±0.19), and(0.88±0.21), respectively, significantly higher than those in the uninfected group(P<0.05). The serum TNF-α, IL-8, IL-6 and PI3 K and AKT proteins of patients with gram-negative bacteria infection were(168.07±37.26) ng/ml,(253.41±49.28) ng/ml,(86.75±18.37) ng/ml and(0.94±0.22) and(1.03±0.24), respectively, significantly higher than those of patients with gram-positive bacteria infection(P<0.001).
