摘 要:目的对比多轴与单轴锁定钢板在SandersⅣ型复杂跟骨关节内骨折中的应用效果。方法将82例SandersⅣ型复杂跟骨关节内骨折患者根据不同手术方法分为观察组和对照组,每组41例。对照组行单轴锁定钢板固定,观察组行多轴锁定钢板固定。比较两组的围术期指标、影像学指标及足部功能恢复情况。结果两组的手术时间、术中出血量、术后VAS评分、住院时间及骨折愈合时间比较,差异不显著(P>0.05)。术后3个月,两组的Gissane角、Bohler角及跟骨高度均高于术前,且观察组高于对照组(P<0.05)。观察组的足部功能恢复优良率为90.24%,高于对照组的73.17%(P<0.05)。结论多轴与单轴锁定钢板在SandersⅣ型复杂跟骨关节内骨折中均能获得较好的固定效果,患者骨折愈合良好,但多轴锁定钢板在维持骨折复位方面更有优势,更利于足部功能恢复。Objective To compare the application effects of multiaxial and uniaxial locking plates in SandersⅣcomplex intra-articular calcaneal fractures.Methods Eighty-two patients with SandersⅣcomplex intra-articular calcaneal fractures were divided into observation group and control group according to different operation methods,with 41 cases in each group.The control group was fixed with uniaxial locking plate,and the observation group was fixed with multiaxial locking plate.The perioperative indexes,imaging indexes and foot function recovery of the two groups were compared.Results There were no significant differences in operation time,intraoperative blood loss,postoperative VAS score,hospitalization time and fracture healing time between the two groups(P>0.05).Three months after operation,Gissane angle,Bohler angle and calcaneal height of the two groups were higher than those before operation,and those of the observation group were higher than the control group(P<0.05).The excellent and good rate of foot function recovery of the observation group was 90.24%,which was higher than 73.17%of the control group(P<0.05).Conclusion Multiaxial and uniaxial locking plates in SandersⅣcomplex intra-articular calcaneal fractures can obtain good fixation effect and fracture healing of patients,but the multiaxial locking plate has more advantages in maintaining fracture reduction and is more conducive to the healing of foot function.
