加味参附汤治疗心阳亏虚型慢性心力衰竭的效果及对患者心室重构、BNP水平的影响

摘　　要：目的探讨加味参附汤治疗心阳亏虚型慢性心力衰竭(CHF)的临床效果。方法采用随机抽签法将2019年1月至2020年1月住院治疗的110例CHF患者分为对照组和观察组,各55例。对照组采用常规西药治疗,观察组在常规西药治疗基础上加用加味参附汤治疗。比较两组的临床疗效及治疗前、治疗12周的凝血功能指标、B型脑钠肽(BNP)及心室重构指标水平。结果观察组的治疗总有效率为96.36%,高于对照组的85.45%(P<0.05)。治疗12周,观察组的血清D-D、Fg、BNP水平均低于对照组(P<0.05)。治疗12周,观察组的LVPWT、LVSS小于对照组,LVFS高于对照组(P<0.05)。结论加味参附汤可改善心阳亏虚型CHF患者的凝血功能及心室重构,降低BNP水平。Objective To explore the clinical effect of modified Shenfu decoction on chronic heart failure(CHF)with heart-Yang deficiency type.Methods A total of 110 CHF patients with hospitalized treatment from January 2019 to January 2020 were divided into control group and observation group by random drawing lottery method,with 55 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with modified Shenfu decoction on the conventional western medicine.The clinical efficacy,the levels of coagulation function indexes,B-type natriuretic peptide(BNP)and ventricular remodeling indexes before treatment and at 12 weeks of treatment were compared between the two groups.Results The total effective rate of treatment of the observation group was 96.36%,which was higher than 85.45%of the control group(P<0.05).At 12 weeks of treatment,the levels of serum D-D,Fg and BNP of the observation group were lower than those of the control group(P<0.05).At 12 weeks of treatment,LVPWT and LVSS of the observation group were less than those of the control group,and LVFS was higher than that of the control group(P<0.05).Conclusion Modified Shenfu decoction can improve the coagulation function and ventricular remodeling of CHF patients with heart-Yang deficiency type,and reduce BNP level.
